Job Summary:
This post is for a STaR worker for our Flexible Assertive Community Treatment Team (FACT)
The FACT Team sits within the Adult Community Mental Health Service (ACMHS) and provides short term assertive interventions to service users under the ACMHS.
This will typically be service users who need support with engagement or need a brief period of increased support.
You will under the supervision of a dedicated FACT clinical lead.* To work as part of a team which provides mental health services.* This post covers all of the North West of Hertfordshire, this includes St. Albans, Harpenden, Hemel Hempstead, Tring, Berkhamsted and the surrounding area. You will need to be able to travel between these places.o To work within the Care Programme Approach / Care Management process with afocus on Recovery Principles.o To provide support, give time and thus promote recovery.
Main duties of the job
* Complete regular visits within the remit of the agreed care plan
* Home visit/interventions offered to all those within the catchment area
* Connected Lives Assessments
* Manage medication with support of the FACT lead or nurse
* Liaise with GP's and Carers/service providers where felt appropriate
* Providing evidence-based interventions
* Work as part of the wider team
* Provide MDT input where appropriate
* Support Service user to complete the Dialog
* Risk assess as part of the interventions offered
* To provide support, give time to an allocated group of service users and thus promote their recovery and promote their place in the broader community.* To ensure that service users engage beneficially with the agreed Care Plan andaccess appropriate services provided on a regular and consistent basis.* To positively promote independent living of service users within community.
