Job summary

We are currently seeking a Community Mental Health Nurse Band 6 or a Social Worker Band 6 with extensive Mental Health Experience, to work within one of our Crisis Resolution & Home Treatment Team (CRHTT) via a Bank Contract, based in Ware for our Acute Mental Health, South East CRHTT Team. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.

This is a specific role within the wider South East CRHTT to provide in-reach and support to our new Crisis House. Working with the wider CRHTT colleagues and professions, the post-holder will predominantly work 5 days per week (in 7, so will include weekend working at times) and in-reach as needed to support service users staying at the Crisis House and our partner agency - who are specialists in Mental Health support in community settings.

This role will specifically involve travel to other HPFT sites for the purpose of 'in-reach' to our inpatient wards as needed.

HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.

All applicants must be registered with Social Work England or hold a valid NMC Statement of Entry and should have completed Preceptorship and can evidence this.

Main duties of the job

The post holder will support the Team Leader in the development and implementation of a multi-professional Crisis Resolution/Home Treatment Team providing a high- quality service to meet the needs of people in acute phases of mental illness, and their carers.

The seven-bedded Crisis house will provide extra support in a community setting for service users from across Hertfordshire. The planned length of stay is for up to 2 weeks and there will be Way-through staff providing 24/7 support and HPFT will have the clinical input of both a Psychologist and Occupational Therapist for part of the week.

The South East CRHTT nurse successful in joining the team will have a role in managing and progressing the referrals into the house and will work with the wider Team in supporting the service users under the team's care - with the catchment being from Royston to Bishop's Stortford to Cheshunt and Waltham Cross.

You will undertake urgent assessments of people with mental health problems in acute crisis and where appropriate provide intensive community-based treatment/support services as an alternative to in-patient admission. The service will be available 24 hours a day, 365 days a year.

The post holder will be responsible for helping to establish and maintain team systems and processes for effective team working and processes, including outcome measures and audits under the direction of the Team Manager.
