Job summary
We are currently seeking an experienced Community Mental Health Band 6 Nurse to work within one of our Children and Young People's Mental Health Services (CYPMHS) via a Bank Contract, based in Borehamwood for our Children and Adolescent Mental Health Services (CAMHS) South Team.
You will be joining our Temporary Staffing Team within the HPFT - Hertfordshire Partnership University NHS Foundation Trust.
This role will include working with young people from the age of 13 up to 18, providing an enhanced community service, within young people's homes, as an outreach service of community specialist CYPMHS service.
HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving
All applicants need to hold a valid NMC Statement of Entry and should have completed Preceptorship and can evidence this.
Main duties of the job
You will be responsible for the clinical and operational service delivery, ensuring individualised comprehensive assessments of Children and Young People (CYP) in the community, and support colleagues in the care management service of complex clinical pathways ensuring it is effectively delivered.
The post holder will be accountable for delivering a quality of service that is supportive, and be efficient to provide advice and support to other members of staff, in line with the organisation's objectives and performance indicators of the Choice and Partnership Approach (CAPA) model.
You will be required to develop and maintain effective working relationships with colleagues within the Trust and other relevant services and organisation, in order to maintain a safe and effective service.
Responsibilities will include the clinical and operational supervision of junior nursing staff and students, as delegated by the team manager.
The post holder must be competent in using IT and have the relevant skills to carry out the activities required for the post

Support the Team Leader in establishing and maintaining effective inter team and inter agency communication.
Provide effective liaison/communication throughout the care pathway to service users, their carers and others, responding sensitively to all needs especially in highly emotive settings
Maintain effective joint work with others, develop positive working relationships alongside the Specialist CYPMH Clinicians and clear communication with Multidisciplinary Team (MDT) and with other external professionals.
Will be able to undertake assessments for service users who meet the service eligibility criteria, and their family. To formulate a care plan and risk assessment, review its effectiveness and re-plan using evidence based interventions to best effect in partnership with the service user.
Will be expected to identify and respond to presentations of managing risk and promote the safety and wellbeing of the service user.
Plan, organise and prioritise resources and daily tasks to meet service and organisational needs
Take a leading role in the day-to-day management of the service supporting the team, including recruitment retention, attendance of mandatory professional meetings, as required.
Document service user care by recording information on electronic care records, demonstrating keyboard skills.
Have access to your own vehicle in order to travel between bases.
Carry out joint assessments with other MDTs as required.
Undertake comprehensive health needs and risk assessments (including capacity assessments) and develop a care plan in accordance with identified needs establishing specialist course of responsibilities/actions based on formulations of the recovery principles.
Provide effective professional supervision/mentorship of junior staff and students assessing proficiencies of practice as required.
Comply with the relevant Trust and service financial policies and procedures
