Job summary
Job Summary:o To work as part of a team which provides mental health services.o To focus directly the on the needs of service users requiring accommodationsolutions using the Support Time and Recovery Model.o To work across service and/or care group boundaries as necessary.o To work closely in association with the Team Leader, other senior staff and otheragencies, to help maximise the quality and quantity of STR services withinavailable resources.o To work within the Care Programme Approach / Care Management process with afocus on Recovery Principles.o To provide support, give time and thus promote recovery.
Main duties of the job
o To provide support, give time to an allocated group of service users and thuspromote their recovery and promote their place in the broader community.o To contribute towards the management of a team of STR workers, by leading onclearly identified, appropriate day to day team management tasks.o Responsible for providing support to the care co-ordinator, in the implementationof a recovery focussed care plan for an allocated number of individual serviceusers.o To ensure that service users engage beneficially with the agreed Care Plan andaccess appropriate services provided on a regular and consistent basis.o To positively promote independent living of service users within community.
