Job summary

We have an exciting opportunity for a Community Mental Health Nurse to join our Adult Community Mental Health Services Team in South East Hertfordshire / Ware

The area is well served with transport links to historic Hertfordshire and its glorious countryside as well as into London and beyond. We are looking for someone who is passionate about delivering care to Adults with mental health issues.

Main duties of the job

  • To support our duty system working alongside our duty worker and duty manager
  • To work as part of a team which provides mental health services.
  • To focus directly on the needs of the service users using the Support Time and Recovery Model.

  • To work across service and/or care group boundaries as necessary.

  • To work within the Care Programmed Approach/Care Management process with a focus on Recovery Principles.

  • To provide support, give time and thus promote recovery.

  • To undertake carers assessment and support of carers identified in the team.

    • To liaise with other agencies and any other relevant professional, in consultation with the Senior STR Worker and Support Services Manager

    • To provide regular and practical support to service users and their carers in developing and managing their dignity and independence.

    • To liaise with external stakeholders and other non-statutory agencies.
