To work as part of the Enhanced Rehabilitation Outreach Service (EROS).
Working alone or with other MDT staff under the supervision of a registered mental health professional, the post holder will be expected:
To work as part of a team which provides mental health services.
The job will be covering the whole of Hertfordshire but your main base will be at St Paul's, Slippers Hill, Hemel Hempstead.
To focus directly on the needs of service users using the Support Time and Recovery Model.
To work across service and/or care group boundaries as necessary.
Carry a keyworker caseload and support coordination of care during the episode of EROS rehabilitation.
Communicate with the Care Co-ordinator, GP and any other involved professionals during the EROS episode.
In conjunction with members of the team, contribute to the development and implementation of the Enhanced Rehabilitation Outreach service to provide a high-quality care and support to service users and carers.
To work closely in association with the Team Leader, other senior staff and other agencies, to help maximise the quality and quantity of support, time and recovery services within available resources.
To work within the Care Programme Approach / Care Management process with a focus on Recovery Principles.

Main duties of the job

To provide support, give time to an allocated group of service users and thus promote their recovery and promote their place in the broader community.
Responsible for providing support to the EROS multi-disciplinary team, in the implementation of a recovery focused care plan for an allocated number of individual service users.
