Job summary

The post holder will work in close collaboration with other professionals, including the wider Multi-Disciplinary Team (Nursing, Psychiatry, Psychology, Allied Health Professionals and Arts Therapies) in implementing care plans that promote service user empowerment through the process. The post holder, with the wider Multi-Disciplinary Team will provide high quality, individualised care, which addresses the needs of the service users and meets individual health needs. Support Nurses with the smooth running of the ward by completing and monitoring the shift planner, providing support , guidance and supervision to health care assistants. Supporting service users to to access appropriate health service/facilities, community facilities and home visits, therefore a car driver is an essential part of the role and holding driving license for manual vehicle.

Main duties of the job

  • Effective and appropriate methods of communication with service users, carers and other professionals.
  • Assist members of the Nursing team and multidisciplinary team with agreed care plans.
  • Support with completion of reports for clinical meetings and support service users to attend and engage in their meetings.
  • To act at all times in safeguarding the integrity, confidentiality and availability of sensitive information relating to both service users and staff.
  • Demonstrate de-escalation skills in managing aggression exhibited by service users.
  • Ability to implement Positive Behaviour Support plans
  • Support in the planning and running of the ward and daily shifts
  • Undertake direct care and support including observation and engagement as per Trust Policy
  • Assist service users with all Activities of Daily Living as required.
  • Assist members of the multidisciplinary team with agreed procedures.
  • Inform the nurse in charge of any potential hazard, accident, incident or service user problem that occurs.
  • Undertake basic housekeeping tasks such as cleaning and tidying to maintain a suitable and safe environment.

OB RESPONSIBILITIES

  • Support the assessment and management of service user care needs through initial data collection, monitoring of service users progress, feedback, discussion, and reports and in care reviews with the clinical
  • Implement agreed plans of care and therapeutic interventions under the guidance of registered practitioners supported by on-going supervision and support.
  • Have informed knowledge and understanding in the psychological, social, physical and spiritual needs of all patients.

PREVIOUS EXPERIENCE & KNOWLEDGE

  • Minimum of 2 years experience as a Health Care Assistant - working in a Health & Social Care setting for people with learning disabilities or complex health.
