Job summary
This is a post for a specialty doctor in the North quadrant of Hertfordshire which encompasses the Stevenage and North Hertfordshire districts of Hertfordshire. The post is based in Saffron Ground, Stevenage and involves providing care to the residents of Stevenage and Knebworth. The population of Stevenage and Knebworth is around 100,000. The estimated Quadrant-wide elderly population of North quadrant of Hertfordshire based on Census 2021 data is 39,000.
This is a newly created post to support crisis function of North SMHTOP. It involves working as a specialty doctor in Saffron Ground, Stevenage and providing diagnostic assessments and treatment to patients with severe, complex, and high risk psychiatric conditions. Although the administrative base will be at Saffron Ground, clinical work may be in another trust location or other community destinations such as at home, care homes, other hospitals. The post holder may be required to attend team meetings at a location determined by the manager.
2 years experience within psychiatry is an essential criteria for specialty doctor posts, should you not hold this experience you may interviewed and considered for an alternative posting which will different in grade and pay.

Initial Assessments
Assessment, diagnosis, formulation of management plan, and sharing decision-making with patients and carers.
Review and re assessments
Flexible review and re-assessment of patients in response to need, and in accordance with the pathway.
SMHTOP meetings
Communication about patients and carers, sharing organisational policies and objectives.
Multidisciplinary reviews
Care programme approach (CPA) reviews and risk reviews.
Clinical advice to team
Regular or ad hoc supervision on clinical matters.
Liaison with colleagues
Discussion about patient care with primary care, secondary care and colleagues in other psychiatric teams.

Meeting the requirements of emergency work, MHA assessments, and assessments of capacity.
Physical health monitoring
Ensure monitoring for metabolic complications related to medication, Lithium monitoring, and adherence to the Clozapine protocols.
Manage caseload
Actively manage outpatient caseloads in accordance with pathway guidance and direction from clinical supervisor. Operate efficient and safe patient review services such as drop in clinics.

The Speciality Doctor will be responsible for the assessment of acute and routine cases, initiating investigations and developing management plans. These duties will be carried out in the SMHTOP Centre or the patients home. You will be well supported by other medical staff as well as a positive and well-functioning multi-disciplinary team.
The Speciality Doctor post is regarded as a core part of the Clinical Team, providing continuity of care and medical back up on site. The post holder will have responsibility for providing a comprehensive range of services in conjunction with the multi-disciplinary team to all patients being managed in the Team. The post holder would be involved in the assessment of emergency work both in terms of new referrals and difficulties with the existing patient population. The post holder will communicate as needed with GPs regarding patients progress, management and discharge summaries after the crisis. The post holder will be expected to provide additional medical leadership to the team and assist with the further development of the service.
The post holder will agree a job plan with the Medical Lead and Supervising consultant. Specialty doctors and associate specialists, like all other psychiatrists, contribute to the assessment and management of the comorbid physical healthcare needs of their patients.
Specialty doctors and associate specialists can play a key role in setting standards within teams, monitoring and delivering these standards ensuring that the clinical care delivered is based on the best available evidence, supporting colleagues, raising concerns within the team to ensure the delivery of high-quality care.
