Job summary

The Nurse assessor will work under the guidance and support of the Band 7 Nurse assessor to assess individuals for NHS Continuing Healthcare, in line with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing care, revised November 2012. We cover the Funded Nursing Care assessments for the East and North locality of Hertfordshire, and our base is at 1 Bowlers Green, Kingsley Green, Harper Lane, Radlett, WD7 9HQ.

The successful applicant will be required to work in close collaboration with other professionals from Adult Care Services, the Integrated Care Board, Nursing Home staff and other relevant care professionals; providing advice and expertise to service users, relatives and the multi-disciplinary team.

We are looking for a dynamic nurse who has a good level of knowledge and understanding of the CHC process and the National Framework for Continuing Healthcare, and who is passionate about the care of our older adult service users.

Main duties of the job

Main Duties and Responsibilities:

  1. Co-ordinate the Funded Nursing Care and/or Continuing Healthcare assessment process for service users, liaising directly with service users/families and other key stakeholders.
  2. To prepare and complete accurate and detailed documentation, which may require presenting service users at CHC panel meetings and feedback outcome to appropriate personnel, service users, family by providing a concise explanation for each decision/outcome.
  3. The assessment/review will start with the completion of the Continuing Healthcare Checklist and proceed to either a full assessment for CHC or an FNC assessment dependent on the outcome of the healthcare checklist. The Nurse Assessor will analyse patient information and complete robust needs assessments to identify and make recommendations for eligibility for NHS Continuing Healthcare or Funded Nursing Care in line with the NHS Continuing Healthcare National Framework relevant at the time of the assessment/ review.
  4. To liaise effectively with the relevant agencies adhering to and implementing policies and procedures for effective CHC throughout the Trust.
  5. To standardise good practice throughout the Trust in relation to CHC.
  6. Identify and promote good practice in services for people without capacity.
