Job summary
An exciting opportunity has arisen for a qualified nurse to join the CRHTT Glaxo Unit. A newly formed multi-professional Crisis Resolution and Home Treatment service providing high quality interventions to meet the needs of service users in acute phases of mental illness in the community, as an alternative to inpatient admission.
You will support the team leader in the development and implementation of the service, embedding therapeutic interventions in to the existing local mental health crisis service.
Ensuring the continued efficient and effective running of the team, providing leadership and supervision within the multi-disciplinary staff group as appropriate.
Main duties of the job
In addition to managing the clinical aspects of a Band 6 Nurse, You will take an active part in the assessment, planning, implementation and evaluation of care for service users in collaboration with the North, East and South East multi-disciplinary Crisis Resolution & Home Treatment service (CRHTT). Liaising with colleagues, ensuring they are fully aware of all aspects of a service users' care and treatment.
You will be able to identify individuals' care needs. Planning, implementing and evaluating care plans in collaboration with the CRHTTs. Maintaining lines of communication with all departments and staff involved in a service user's care. Reporting on progress and effectiveness to the appropriate MDT.
Participating in clinical reviews and case conferences, etc. Preparing reports as required. Acting as key worker to an individual or group of service users, taking responsibility for the formulation of their appropriate care plans.
You will also be actively involved in the development and facilitation of group activities with you colleagues.
