Main duties of the job
The successful candidate will:
- be responsible for the delivery of care to an allocated caseload
- implement, monitor and evaluation of care
- administer and/or supervise the administration of prescribed medication
- liaise with other professionals including GPs and external agencies
- ensure full compliance with aftercare issues such as Social Care requirements
- receive full support and training
