Job Summary
Clinical
Practice
Practice autonomously as an
advanced practitioner within the COMPASS service to provide patient-centred
clinical care, encompassing the skills of advanced clinical assessment,
examination, diagnosis and treatment within an agreed scope of practice.
Demonstrate a high level of
expertise within the COMPASS service providing advice, education and support to
staff, patients, their families and carers.
Utilise their expertise and
decision-making skills to inform clinical reasoning to appropriately treat
patients, resulting in the safe management and appropriate referral, admission
or discharge of patients.
Co-ordinate timely early
supported discharges from ED, acute wards in supporting safe transfer of care
from acute to community settings by in-depth assessment and co-ordination
between acute and community.
To be pivotal in encouraging and
managing appropriate referrals within the specialty, attending complex case
discussions, MDT and other meetings.
Work autonomously as an Advanced
Practitioner within the COMPASS service, managing a caseload of patients
delivering individualised direct patient care.
Lead in the development and
expansion of the COMPASS service within the specialist area with management
responsibility for additional team members.
Responsible for identifying and
assessing complex clinical and non-clinical issues, implementing resolutions
and monitoring/auditing outcomes.
Regularly manage expected and
unexpected clinical events relating to Specialist Palliative Care emergencies
requiring high but unpredictable levels of physical effort according to patient
dependency/clinical need.
Support patients, carers and
staff during difficult situations arising in the clinical area, e.g. breaking
bad news or following an unexpected event.
Leadership
and Service Development

Have responsibility for leading
the on-going development of clinical practice and standards of care within the
COMPASS service, including the development of policies, procedures, protocols
and guidelines in collaboration with multidisciplinary colleagues.
Will contribute towards
corporate objectives, acting within local, Trust and statutory guidelines and
policies always
Contribute to specialist forums
and National initiatives.
Provide reports to the Service
Lead as requested.
Support the ED nursing team in
establishing and maintaining effective communication channels with
multi-disciplinary team, patients and their carers and external agencies.
Managing human resources issues,
undertaking PDRs.
To use excellent prioritising
and time management skills to meet the unpredictable and conflicting needs of
the service.
To develop own managerial and
administrative functions in conjunction with role developments
Act as a clinical role model for
developing and delivering care that is responsive to changing
requirement.
Utilise advanced knowledge and
skills relating to Specialist Palliative care to provide specialist/generalist
advice to other members of the MDT based on patient assessment.
Receive referrals via a variety
of sources and manage own caseload and clinical priorities according to agreed
protocols and working practices
Within scope of practice and
clinical competence request and / or undertake diagnostic procedures and
clinical investigations related to plans of care
To provide follow-up support for
patients within the community by facilitating an appropriate referral to
primary care teams if required and ensuring patients are aware of services
available.
To be seen as a clinical expert
and act as a resource to all members of the multi professional teams working
within the Trust
Undertake training to develop
further advanced clinical practice roles required by the service to provide a
high standard of patient care.
Participate in professional
nursing initiatives as an invited member to the Directorate and trust nursing
groups eg Fundamentals of Care Committee, DREAMS
Investigation of complaints
relating to patient care, leading the development of action plans to address
areas of concern, identifying learning and changes to practice.
To maintain and promote
evidence-based practice by participating in and disseminating research.
To ensure National policies,
initiatives and standards are adhered to within the service.
Ensure that high standards of
documentation are maintained with accurate, complete and up to date information
regarding patient care are kept in accordance with Trust standards.
By utilising service improvement
and a variety of other techniques to monitor and instigate change to
continuously provide a high-quality service.
To co-ordinate, analyse and
implement results of audit to lead changes in practice on a local and regional
basis.
To act as a positive role model
for colleagues within the area.
To provide professional
leadership for the nurses within the service.
Monitor team dynamics,
initiating action to enhance team effectiveness where appropriate.
To demonstrate ability to review
and negotiate available resources to enhance practice.
To demonstrate ability to manage
resources effectively.
Engage in recruitment selection
and orientation of new staff including participation in a robust Divisional
Trust programme to train/develop new staff.
Education
and Training
Act as an expert educational
resource for clinical staff, patients and carers by providing formal and
informal education; promote and develop a positive learning environment.
Work with the senior nurses
within the Trust to support a Trust-wide development for Advanced Clinical
Practice roles for Health Professionals (supports Trusts Clinical Strategy).
Instrumental in educating teams
regarding the COMPASS service and improving their care.
Candidate must have completed an
MSc in Advanced Clinical Practice or Band 7 Development role on completion of a
Masters undertake as part of a Band 7 would achieve Band 8a.
Recognising the limitations of
their own professional practice and competence and undertake further training
and academic qualifications as required to maintain own specialist knowledge.
Support staff undertaking
non-medical prescribing course.
Research
and Evidence-Based Practice
Critically appraise and
implement evidence-based practice.
Contribute to research, audit,
and COMPASS service evaluation projects.
Disseminate findings to inform
practice and service development.
Identify areas for nursing
research, sources of funding and initiate research projects.
Professional
Requirements
Registered healthcare
professional with NMC/HCPC/GPhC) registration.
Evidence of working at an
advanced level of practice, in line with the HEE Advanced Practice Framework.
Masters degree in advanced
Practice or relevant clinical specialty or Band 7 development role
Ability to work autonomously,
demonstrating critical thinking and complex decision-making.
Evidence of leadership,
education, and research engagement.
Commitment to ongoing
professional development and maintaining professional registration.
