Job summary
An exciting opportunity has arisen for a motivated and enthusiastic individual to join our Haematology Team at Worcestershire Acute Hospitals trust as a Haematology Support Worker.
The Haematology Support Worker will assist the Clinical Nurse Specialist team in providing specialist, high quality expert care and support for patients (and their carers) with Haematological conditions, delivering holistic, patient centred care according to their changing health care needs. They will also assist in the tracking of patients on the Patient Centred Follow Up (PCFU) Programme and provision of holistic needs assessments for patients within the service.
The applicant should have excellent communication skills and experience working with multiprofessional teams

Main duties of the job
To work under the direction of the registered nurse providing high quality person centred support for Haematology patients, particularly those on the Haematology Patient Centred Follow-up (PCFU) Programme
The role involves supporting aspects of care delivery, coordination of the patients pathway, assessment of patient/carer needs and the provision of general information, help and support to those within the Haematology Patient Centred Follow up programme. To be an extended member of the Haematology cancer MDT, ensuring effective communication across the Trust, Tertiary providers, Primary Care and statutory and voluntary agencies
