Due to a member of the Practitioner Team going on maternity leave, an opportunity has arisen for a dynamic and enthusiastic individual to join the Team for a fixed 12-month contract, and support the implementation of Martha's Rule across the organisation to ensure that patients and families have a clear route to escalate concerns.
As a Practitioner, the postholder will work as part of the Hospital Outreach 24/7 Practitioner Team and will respond quickly and effectively to deteriorating patients on the wards. They will work alongside Multi-Disciplinary Teams (MDTs) using the Prevention, Identification, Escalation and Response (PIER) Model to reducing physical deterioration. They will also have responsibility for the daily management of a caseload of patients, in collaboration with the medical teams.

Main duties of the job
This post is a key position and as such responsibilities will include:
Work towards health promotion and prevention, and comprehensively assess patients for risk factors and early signs of illness and deterioration.
Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, and provide direct support to patients and clients.
Develop the ability to perform comprehensive patient assessments based on history taking and physical examination of acutely unwell adult patients to arrive at a different diagnosis.
Undertake appropriate investigations and provide supportive measures to prevent patient deterioration whilst awaiting medical review or senior review.
Work collaboratively with ward-based staff, demonstrating the use of evidence-based specialist skills required to develop and deliver a detailed management plan for the critically ill ward patient.
Ensure agreed medical/nursing treatment plans are implemented, promptly reporting and recording any changes in the patient condition.
Work collaboratively with other members of the Hospital Outreach 24/7 Team and Critical Care MDT to develop local service delivery.
This post will be split between Worcestershire Royal Hospital and The Alexandra Hospital, Redditch and travel across the county is essential.
