Key Duties:
Work autonomously as an advanced practitioner within the speciality, managing a caseload of patients delivering individualised direct patient care
Assist in the development and expansion of the Advanced Clinical Practitioner service within the specialist area
A knowledgeable team leader, supporting staff, service and policy development within a defined area/specialty.
Provide and maintain expert knowledge and practice in venepuncture, cannulation, central venous devices, chemotherapy drugs and biological therapies
Responsible for identifying and assessing complex clinical and non-clinical issues, implementing resolutions and monitoring/auditing outcomes.
Undertake highly specialised technical and/or practical skills to ensure optimum patient management including; complex patient monitoring; chemotherapy reviews and assessments; equipment management and defined expanded clinical roles e.g. PICC line placement.
Accountable for own actions in accordance with Code of Professional Conduct. Will contribute towards corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
Highly developed communication skills enabling maintenance of high quality service and ensuring timely management and resolution of unexpected events and situations.
Contribute to specialist forums and National initiatives.
Identify areas for nursing research; sources of funding and initiate research projects.
Regularly manage expected and unexpected clinical events relating to SACT requiring high but unpredictable levels of physical effort according to patient dependency/clinical need.
Support patients, carers and staff during difficult situations arising in the clinical area, e.g. breaking bad news or following an unexpected event.
Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
General Duties:
Clinical / Professional:
Direct responsibility for assessment, examination, investigation and diagnosis of patients receiving SACT.
Utilise their expertise and decision making skills to inform clinical reasoning to appropriately treat patients, resulting in the safe management and appropriate referral, admission or discharge of patients.
Utilise advanced knowledge and skills relating to SACT to provide specialist advice to other members of the MDT on the basis of patient assessment.
Receive referrals via a variety of sources and manage own caseload and clinical priorities according to agreed protocols and working practices
Within scope of practice and clinical competence request and / or undertake diagnostic procedures and clinical investigations related to plans of care
To provide follow-up support for patients within the community by facilitating an appropriate referral to primary care teams if required and ensuring patients are aware of services available.
To provide specialist advice to the chemotherapy multi-professional team to facilitate high quality patient care.
To be seen as a clinical expert and act as a resource to all members of the multi professional teams working within the Trust
Develop and utilize interventions deemed beneficial to patient care e.g. Telephone clinics, face to face ACP led clinics, aimed at improving the experience of cancer, for patients and families.
To be aware of current trials being conducted within own specialty area and be able to inform patients about these. To be aware of new trials coming on line and the impact these trials will have on the Trust service provision.
To regularly audit the service so patient care is continuously improved and to ensure documentation and record keeping is comprehensively maintained
Undertake training to develop further advanced clinical practice roles required by the service in order to provide a high standard of patient care
Educational:
Participate in the educational and professional framework for the development of advanced practitioner roles within oncology and haematology (supports oncology and haematologys clinical strategy).
Act as an expert educational resource for clinical staff, patients and carers by providing formal and informal education; promote and develop a positive learning environment
Lead and actively participate in the delivery of educational programmes for all grades of nursing, medical and allied health professional staff
As a Non-medical prescriber participate in development of forums to support with dissemination of best practice and highlight concerns in relation to the role.
Work with the senior nurses within the trust to support a trust wide development for advanced clinical practice roles for health professionals (supports trusts clinical strategy).
Candidate must have completed an MSc in Advanced Clinical Practice or equivalent, recognising the limitations of their own professional practice and competence, and undertake further training and academic qualifications as required to maintain own specialist knowledge.
Maintain close links with local HEIs and participate in the development and delivery of new and established advanced practice programmes.
Clinical Governance:
To maintain and promote evidence based practice by participating in and disseminating research.
To ensure national policies, initiatives and standards are adhered to within the service.
Ensure that high standards of documentation are maintained with accurate, complete and up to date information regarding patient care are kept in accordance with Trust standards
By utilising service improvement and a variety of other techniques to monitor and instigate change to continuously provide a high quality service.
To co-ordinate, analyse and implement results of audit to lead changes in practice on a local and regional basis.
To co-ordinate in the development, implementation and review of policies within the service and on a wider trust basis as appropriate.
To undertake investigations pertaining to untoward serious incidents/complaints.
Patient / Carer Support / Involvement:
Provide a seamless, high quality service for patients and carers from referral through to assessment, diagnosis, treatment and review.
To ensure that patients and carers are aware a specialist contact is available for patients 24 hours a day to meet their needs whilst undergoing SACT.
To actively seek patients feedback on the service provided using a variety of techniques and to utilize this information to instigate change in service provision.
To ensure patient information is accurate, consistent and reflects current and clinical practice.
Elicit, interpret and implement local and national patient/user information and involvement initiatives.
Strategic / Service Development:
To lead and develop a defined area of clinical practice within the SACT service, promoting inter-disciplinary team and collaborative working practices
Develop the role and service according to changing patients needs, service requirements and evidence based practice.
To attend and contribute to relevant meetings internally and externally with primary groups representing Worcestershire Acute Hospitals NHS Trust, ensuring the Trusts views, influences developments and the local health economy views are represented.
Maintain working knowledge of local and national professional policy and strategy
To develop policies, guidelines and protocols to support the on-going development of the service in line with local and national developments.
Leadership / Management:
Provide highly visible and accessible professional leadership and demonstrate expert knowledge and standards of clinical practice
Meet regularly with team members as a team and individually as required to support their personal and professional development
Lead and actively participate in service / developmental projects, quality initiatives and statutory accreditation processes
Demonstrate effective leadership skills supporting the senior management team (Lead Chemotherapy Nurse / Matron / Directorate manager) in service development and lead on implementation of any changes to the service.To act as a positive role model for colleagues within the area.
To provide professional leadership for the nurses within the service.
Monitor team dynamics, initiating action to enhance team effectiveness where appropriate.
To act as a professional leader within the specialty, organization and across the wider healthcare setting.
To demonstrate ability to review and negotiate available resources to enhance practice.
Engage in recruitment selection and orientation of new staff including participation in a robust Divisional Trust programme to train/develop new staff.
To provide and support written evidence of achieving certain specified competencies for new staff

To maintain personal and professional development by attending appropriate training / educational opportunities
Practice in accordance with the Professional Body Code of Conduct and applicable legislation.
