Oncology Clinical Support Worker/Navigator

Job summary

An exciting opportunity has arisen for a compassionate and motivated Support Worker/Navigator to join the Worcestershire Acute Hospitals Oncology Clinical Nurse Specialist Team. We are looking for an individual who is passionate about delivering high quality person centred support for Oncology patients and also play a key role in the development and delivery of a new Patient-Centred Follow-up (PCFU) programme.

We are keen to hear from an individual who has the necessary drive, motivation and initiative to support and implement this service delivery for 12 months fixed term contract/secondment.

The post holder will support the clinical teams in coordination of the patients pathway, assessment of patient/carer needs and the provision of general information, help and support to those within the Oncology Patient Centred Follow up programme.

The ideal candidate will have an understanding of cancer targets, patient pathways, care plans and holistic needs assessments. We are looking for a confident, enthusiastic and conscientious team player with excellent time-management and organisational skills who can work autonomously and well under pressure. The post holder will need excellent communication skills and an ability to deal with complex situations with service users and clinicians over the phone and in person.

Main duties of the job

To work under the direction of the registered nurse providing high quality person centred support for Oncology patients, particularly those on the Oncology Patient Centred Follow-up (PCFU) Programme

The role involves supporting aspects of care delivery, coordination of the patients pathway, assessment of patient/carer needs and the provision of general information, help and support to those within the Oncology Patient Centred Follow up programme.

To be an extended member of the Oncology MDT, ensuring effective communication across the Trust, Tertiary providers, Primary Care and statutory and voluntary agencies.
