Job summary

This is an exciting opportunity for a motivated Paediatric Nurse with an interest in Diabetes to join our well established and dynamic team in Worcestershire.

The service is delivered from 3 localities, Worcester, Redditch and Kidderminster with the vacancy in our Worcester team.

The role encompasses supporting families and young people with diabetes in managing their long term condition from diagnosis through to transition to the Adult diabetes service. The post holder will manage a caseload with support from a senior PDSN and make autonomous clinical decisions in managing treatment for an individual.

Our service was an early adopter of hybrid closed loop insulin pump technology and there will be lots of opportunity to gain knowledge and experience in this field.

The team offers additional activities for our CYP and families outside of the clinical environment, such as a teddy bears picnic, Christmas parties, Teen Day and an activity weekend. This provides peer support as well as the opportunity for structured education.

Main duties of the job

The post holder will show excellent communication, presentation and teaching skills. You will be able to work within the existing Multidisciplinary team to provide a seamless, cohesive service to our current population of children and young people with diabetes. You will be focused on improving care and demonstrate that you are able to contribute to ongoing service development.

The post holder will be responsible for supporting the day to day needs for children and young people with diabetes who may use multiple daily injections, hybrid closed loop pumps and continuous glucose monitoring. The post holder will provide nursing support in MDT clinics and nurse led clinics, which may be face to face, virtual or telephone calls.

You will be expected to deliver educational programmes to young people and families and also to teach the wider medical and nursing community in paediatric diabetes. Your role will also include training school/nursery staff and other carers to learn the key skills required to deliver appropriate, safe care for young people within educational or residential settings.

There will be an expectation that you will provide support and clinical supervision for more junior members of the team and wider acute paediatric staff.

The post holder will continue their professional development with access to regular training and education to maintain and enhance their skills as diabetes technology evolves.

Job Purpose:

To work as a clinical nurse specialist in paediatric diabetes specialist care across the accepted domains of the role as a clinician, educator and researcher as part of the Countywide Paediatric Diabetes Team.

To support the multidisciplinary team in line with the Best Practice Tariff, National Service Frameworks, government and Trust Initiatives for Paediatric Diabetes Care by being a key contributor to the on-going development of the service.

To work as a member of the paediatric specialist diabetes care team, to ensure a cohesive, holistic family centred, and research-based approach to the management of diabetes in children and young people.

To participate in teaching programmes to parents/carers, children and young people and school/nursery staff.

The role will be innovative and encompass all aspects of the patient care pathway, acting as a nurse advisor to support children and young people with diabetes, their families and other carers within a variety of settings.

Key Duties:

To autonomously manage a clinical caseload of young people with diabetes and their families. Providing child focussed, holistic family centred care in various settings, including Acute Trust sites, home or community and educational establishments. To instigate and adjust treatment within set guidelines.

To be a resource for all parties involved in a childs care including primary and secondary care staff, those in education and social care.

To undertake nurse led clinics and baseline assessments, formulating clinical management plans.

To undertake specialist investigations, interpret results and initiate appropriate action.

To provide emergency support to families at times of crisis such as illness management and to participate in an on-call service when required
