Job summary
As Consultant Nurse Practitioner for the
Hospital Outreach 24/7 Team, the postholder will work strategically
across the Deteriorating Patient and Sepsis pathways, influencing other disciplines
and the wider organisation and ensuring organisational and professional boundaries
to support the implementation of Marthas Rule.
The Consultant
Nurse Practitioner will mentor the Nurse Practitioners and support the
education and training needs of the Team in the recognition and treatment of
deteriorating patients.
Key components of this post include the provision of expert
clinical care, strong clinical leadership, and developing and implementing a
strategy for development of the sick, deteriorating and sepsis including the
co-ordination of research, audit and quality improvement activity.
Due to the
recent Management of Change and the amalgamation of the Hospital @ Night Team
and the Critical Care Outreach Team, this has created the opportunity for a
dynamic and enthusiastic individual to lead the Hospital
Outreach 24/7 Team and support the implementation of Marthas Rule across the organisation to
ensure that patients and families have a clear accessible route to escalate
concerns.
Specialised Clinical Services Division (SCSD) is

Main duties of the job
This post is a key position and as such
responsibilities will include
Ensuring that patients receive care that is evidence-based where
possible and in accordance with the National Guidelines and having significant
input into the implementation of national and local healthcare policies and
into developing innovative ways of delivering the service.
Working with all health care professionals to ensure patients are
identified and given early treatment to improve outcome.
Developing the life after critical care illness service, providing
follow up care to patients who have been critically unwell and to make
referrals to other professionals where and when appropriate
Providing
innovative professional leadership in order to ensure and consistently develop
a high quality, safe and cost-effective service for patients.
Leading the professional
workforce agenda and maximise its contribution to the achievement of the Trust
and Divisional strategic goals. The post holder will ensure that this is
achieved in accordance with the patients needs and nationally determined and
locally developed professional standards.
This post will be
split between Worcestershire Royal Hospital and The Alexandra Hospital and travel
across the county is essential. The
postholder will be responsible for holding the Senior
Nurse Bleep providing out of hours support when on-call.

Trust, aiming to reduce unplanned admissions to our critical care units whilst
providing expert clinical and leadership for the further development of the
service.

and implementing a strategy for development of the sick, deteriorating and
sepsis including the co-ordination of research, audit and quality improvement
activity.
The postholder is responsible for ensuring
that patients receive care that is evidence-based where possible and in
accordance with the National Guidelines.
The postholder will be expected to have
significant input into the implementation of national and local healthcare
policies and into developing innovative ways of delivering the service. This will include developing collaborative
relationships with other local units both inside and outside the Trust as
appropriate to the needs of the service.
The postholder will work directly with
patients, clients, and communities for more than 60% of their time.

Be a role model undertaking the expert
practice component of the role alongside students (medical and nursing) or
nurses who are inexperienced in nurse-led care.

skills and knowledge to comply with the NMC standards for nurses, including
planned consideration of Advanced Practice roles.
Collaborate with other members of the
SCS Divisional Management Team and to act as a resource for end-of-life care.
