Job summary
As a Hospital diabetes Specialist Nurse, you will provide advice to clinical teams to ensure people living with diabetes have good diabetes care while in hospital. You will work closely with your community colleagues to provide seamless care on discharge. You will support the NADIA harms audit and report any issues with care to the trust via its clinical safety platform.
Education is fundamental in diabetes management so you will be involved in all areas of diabetes education from 1one top one patient and family education to groups of people with gestational diabetes and the education of health care professionals. You will support student nurses on placement and spoke days. You will also support registered nurses with diabetes education days and link nurse days. you will also provide education when teaching moments arise in clinical areas.
You will manage a caseload of patients and adapt to changes in the working day from new admissions and advice and support to prevent admissions.

This is a part time role of 0.8 WTE and will be shared with another team member.
You will need to be able to travel across site to support care at all of our trust sites.
Main duties of the job
Provide support to hospital teams to manage people with diabetes safely while in hospital.
Support people with diabetes who are pregnant or develop gestational diabetes. This will involve face to face and telephone appointments and group teaching.
Support Student nurses on placements or spoke days. Ensuring they have a wide range of opportunities to see what a role as a clinical nurse specialist involves.
Provide education to a variety of health care professionals from Healthcare Assistants to Consultants.
People newly diagnosed with diabetes or who have had a significant change in their health status need additional support and education. You will be that point of contact to provide them with what they need to manage independently with their diabetes in the community. You will advocate for your patients and support them to achieve good glycaemic control.
You will provide support and be a link in the Multi-disciplinary foot team and support these patients on inpatient and outpatien basis.
You will support the team leaders to develop guidelines and policies for use in the hospitals. You will participate in local and national audits.
