GYNAECOLOGY SERVICES

The Gynaecology Service is delivered by multidisciplinary speciality teams with Consultant Speciality Leads for all sub-specialisms (i.e. Ambulatory gynaecology, Acute emergency

gynaecology, Fertility, Endometriosis, Oncology, Urogynaecology and Colposcopy) strongly supported by a Gynaecology Matron and Specialist Nurses in Gynae Oncology, Ambulatory Gynaecology, Colposcopy, Infertility, Endometriosis and Urogynaecology. In addition, we have an Advanced Scrub Practitioner and Senior Nursing staff in the Early Pregnancy Assessment Units. There is a dedicated Clinical Director for Gynaecology.

All acute care for gynaecology is on the Worcestershire Royal Site with a newly dedicated Emergency Gynaecology Assessment Unit on Mulberry Suite. Outpatient gynaecology services are delivered at the three main hospital sites and at four community hospital sites (Evesham, Bromsgrove, Malvern and Tenbury Wells).

At the Alexandra Hospital, Redditch (AHR) we have developed a Womens Health Unit where we undertake most ambulatory gynaecology procedures.

We have moved successfully to a Hot site - Cold site model for Gynaecology Surgery with Day case and short stay intermediate procedures now performed solely at KTC and AHR. The most complex gynaecology surgery occurs at the AHR and WRH site.

In line with GiRFT priorities we are delivering increasing numbers of same day discharge gynaecology surgery and are a regional leader in same day discharge total laparoscopic hysterectomy.

There are close links between the Obstetrics and Gynaecology Directorate and the otherDirectorates within the Trust. We have excellent links with the Urology department who have led the trust in the delivery of Robotically Assisted Surgery. The Gynaecology Department, fronted by the Endometriosis and Gynae Oncology Consultants, have recently completed their 100th procedure on the robot. We also have a superb Colorectal team who have excellent laparoscopic capabilities and are also developing in Robotically Assisted Surgery with joint complex endometriosis cases planned in the near future. The Vascular surgical team and interventional radiology team are based at WRH and are always available for advice and support where required.

We were one of the first BSUG accredited units for Urogynaecology and we were successfully re-accredited in 2018.

The Unit has also been accredited as a BSGE Endometriosis Centre and remains one of the busiest within the country. There is wide clinical expertise in advanced laparoscopic surgery within the Directorate and wider Trust.

The Gynaecology Oncology service is further supported by the Worcestershire Oncology Unit which is a state of the art radiotherapy centre, housing 3 linear accelerators.

There are weekly countywide MDT meetings for the Gynaecology Oncology Team, which are well supported by the MDT coordinators, specialist nurses, histopathology, radiology andoncology Consultants. In addition, there are bi-monthly MDTs for Colposcopy.

Monthly MDTs also run for the Endometriosis team, with bimonthly MDT running for the Urogynaecology Team and weekly for the Fertility team.

MATERNITY SERVICES

The Maternity Services were rated Good by the CQC when inspected in 2023. It is now everyones ambition to continue to develop and improve and become Outstanding in the next inspection.

We have approximately 5000 deliveries per year at Worcester. The labour ward has 10 delivery rooms (including a pool room), 3 observation rooms and 2 bereavement rooms. It also contains one Obstetric Theatre and one Obstetric Intervention Room, the management of which has recently been taken over by SCSD who provide cover for scrub nurses throughout the week. We have caesarean section lists 4 days a week with 2 sessions per week covered by a dedicated consultant.

We also have a midwife led unit; the Meadow Birth Unit located close to the Delivery Suite on the Worcester site which has 4 delivery rooms. The team were awarded National Birth Centre of the Year in 2017.

Approval has been gained for redevelopment of Maternity Triage, Delivery Suite and the Intervention room (currently used as the second Obstetric theatre) as part of the Acute Services Review (ASR) and building work is anticipated to commence later this year. This will enable us to have a Second Obstetric theatre 24/7.

We use BSOTs (Birmingham Symptom Specific Obstetric Triage System) within Maternity Triage and have recently implemented a Telephone Triage Midwife.

The Obstetric service is also enhanced by a Level 2 Fetal medicine service and excellent bereavement and debrief support services with specialist midwives in post another award-winning service. There is a dedicated Clinical Director for Maternity.

The special care baby unit at Worcester is a Level 2 Unit and is adjacent to the Delivery Suite. It has 4 Neonatal Intensive care cots. We accept in utero transfers of mothers who are in premature labour or who require premature delivery from 27 weeks gestation for singleton pregnancies and 28 weeks for multiple pregnancies. Below these gestations babies are mainly transferred to the regional tertiary unit at Birmingham Womens Hospital.

There is close communication and excellent mutual support between the Obstetric and Paediatric staff and there are weekly Obstetric Case Review meetings, alternate month Perinatal Mortality & Morbidity meetings (PMM) and a newly created Perinatal Meeting running alternate months to the PMM.

Support is provided by medical colleagues (cardiology, renal, endocrinology, neurology, haematology and rheumatology) for our complex maternal medicine patients with monthly cardiac and maternal medicine MDTs.

We work closely with Wye Valley Trust, Hereford, in our LMNS (Local Maternity and Neonatal System) and our Maternity and Neonatal Voices Partnership (MNVP). Obstetricians support Hereford with the provision of preterm and maternal medicine cover and there is potential to expand this service further.

The communication between the units and the tertiary centre at Birmingham Womens Hospital is extremely good. There are close working links across the Maternal Medicine Network, the fetal medicine service and the genetics, cytogenetics and neonatal pathology services. We also have close working relationships with radiology and urology and manage complex placenta percreta cases within the Trust.

The Maternity service utilises the Badgernet Maternity Information System and we have 2 dedicated Digital midwives.

Antenatal clinics are held daily at WRH, where there are 3 dedicated ultrasound machines for the antenatal clinic and a further machine for use on Delivery Suite. There are antenatal clinics 3 days per week at the AHR, 3 days per week at KTC and weekly in community hospitals at Evesham and Bromsgrove. There are also Obstetric Day Assessment Units (DAU) open Monday Friday at AHR and KTC and 7 days of the week at WRH.

In Kidderminster the antenatal clinic is positioned alongside the gynaecology clinic and both have been extensively refurbished. Pre-assessment clinics also take place within this facility. At KTC there are 2 designated scan rooms within the antenatal/gynaecology clinic with facilities for transvaginal scanning.

DUTIES OF THE POST

Under the guidance of the consultants, the post holder will be responsible for the clinical care of Obstetric and Gynaecological patients. The post holder will work with all of the consultants in the Directorate.

We have 2 teams on call one for Obstetrics and separate team for Gynaecology (consultant, registrar and junior tier)

To assist at Out-patient clinics (Antenatal and general Gynaecology).

To participate on call as part of the middle grade rota 1:8

Perform surgery in obstetrics and gynaecological within his/her capability without supervision and if difficulties arise beyond his/her competence to consult the relevant or on call Consultant.

To be responsible for the maintenance of proper case records and investigations on patients, and correspondence with General Practitioners and others as required

In our department opportunities exist for Clinical Fellows to develop and train in Ambulatory Gynaecology (comprehensive service providing outpatient hysteroscopy, Myosure and endometrial ablation), Colposcopy, endometriosis management, infertility, Maternal Medicine and Urogynaecology. We have well established multidisciplinary endometriosis and pelvic pain clinics, Fetal medicine scan clinics and diabetes clinics.

Duties will be arranged with the consultants to whom you are responsible, and you must discuss and understand these duties as soon as possible as you take up your appointment. The consultant has final responsibility for all patients under his/her care and therefore any clinical problems causing anxiety should be referred to the Consultant, as the senior member of the team, or in his/her absence, the Consultant covering his/her duties.
