We would like to extend an invitation to join our well established Urology CNS team, as a Band 7, (part time 22 and half hours), if you feel you have the right skills and qualities. To work clinically as a urology nurse specialist providing a range of nurse led clinics, support the service and support the department with expert knowledge and skills. Identifying and directing patient referrals from the GP and other sources to the most appropriate clinic. To provide a high standard of evidenced based care for patients with urological conditions. To provide teaching and clinical expertise across the hospital sites and community teams with a passion for improving quality. We currently are able to offer 1 part time post.

Main duties of the job

Main Duties

To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries. For both benign and cancer urological conditions.
To undertake nurse led clinics and to be responsible for excellent standards of nursing care to patients through the planning, co-ordinating, delivering and evaluating nurse-led clinics.
To work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are research based and which enhance the quality of patient care.
To contribute to the facilitation of the planning, delivery and evaluation of care pathways for a designated group of patients.
To contribute to training and education within specialist area.
To promote an innovative and progressive attitude to the continual improvement of patient care through reviewing the evidence base and embedding this in practice through engagement in research activities.
To act as a visible role model and expert practitioner.

To be able to perform diagnostic procedures eg flexible cystoscopies, urodynamics assessment, this is not an exhaustive list

Job responsibilities

The CNS will work alongside Consultant Urologists in supporting men referred to the Trust with suspected Prostate Cancer via Cancer 2-week rule. Through the principles of the Best Practice Prostate Pathway, it is anticipated that men on this pathway will have an improved waiting time to diagnosis with a positive patient experience. The CNS will also work along site Consultants and support the Urology cancer and benign services.

The post holders will work closely with the Consultant Urologists in delivering pathways for benign service and suspected urological cancers. This job description outlines the current main responsibilities of the post. However, the duties of the post may change and develop over time and this job description may, therefore be amended in consultation with the post holder.

The CNS will work effectively as an autonomous practitioner and as a core member of the multi-disciplinary team within Urology to provide the highest possible standards of care for patients with urological disease.

The CNS will work with the support and direction of the head of nursing for surgical specialties, the matron, the Trust lead cancer nurse and the Consultant urologists to improve the care of patients, provide specialist help advice and support to patients and their families. You will act as an advocate by providing information which leads to informing patient choice. You will use the views of patients and carers to inform service improvement.

The post holder will work as an integral member of the Multidisciplinary Team across the Worcestershire Acute Hospitals, collaborating with a range of health care providers. The post holder will provide highly developed specialist nursing, Education, Research and Audit, Leadership and Consultancy and a Professional Role model.

EXPERT CLINICAL PRACTICE:

1. Provide specialist nursing practice intervention so that the specific needs of individual patients / clients are addressed. Provide direct care as per job plan, working alongside the nursing staff in the clinical area- providing a visible and strong role model to ensure best practice

2. Conduct independent nurse led clinics for a variety of urological conditions that need monitoring/treatment or review post-surgery. Work autonomously within the parameters of their competencies to make decisions and adjustments to patients treatments in line with their needs. Tasks may include, altering medication, requesting imaging and appropriate blood tests as well as referring back to the consultant or other multi-disciplinary members if further advice is needed.

3. To provide advice and support for all members of the multi- professional team in the care of patients undergoing diagnostic pathway with suspected urological cancer. To liaise closely with members of the multi professional team so that patients with confirmed urology cancer experience a seamless, high quality service.

4. To provide continence advice.

5. To provide guidance, support, advice and counselling to patients and their family/ carers along the patients pathway. To know when to refer on to other professionals for more specialist guidance, support, advice and counselling.

6. To promote patients autonomy through education and support so that the individual is able to make informed choices about their own care.

7. To act as the patients advocate when required and liaise with the multidisciplinary team as necessary.

8. To act as a role model of expert clinical practice, empowering others through effective role modelling, teaching, and team working.

9. To develop guidelines and protocols to deliver comprehensive evidence-based service for patients.

10. To recognise opportunities within the urology service for further development and expansion, to facilitate and manage change within defined clinical and organisational boundaries.

11. Working as an integral part of the multidisciplinary team to develop the service and appropriate referrals. To develop effective working relationships with all other departments resulting in effective coordinated care for clients, with all other local, regional and national organisations

12. To be able to work both individually and as part of the multidisciplinary team.

13. To case manage a team of patients in partnership with the multi-professional team.

14. To set, monitor and review standards on a regular basis in accordance with clinical governance essence of care benchmarks and the NHS plan. To liaise with MDT and Matron re: leading on patients satisfaction surveys and present findings at Clinical Governance and other appropriate meetings.

15. To be able to lead on developing integrated care pathways and nursing care pathways in liaison with members of the multidisciplinary team.

16. To develop own nurse led clinics in conjunction with the Consultants and the out patients department, after consultation with the Matron and key stakeholders.

17. To maintain nurse led and telephone clinics whereby patients are provided with information and support and the problems and needs of patients and their careers are assessed and appropriately channelled and referred to other services as required.

18. In collaboration with patient information services write new patient information and update existing patient information.

EDUCATOR

1. To identify assess and meet the educational needs of the patients and their family

2. To identify the educational needs of the multi-professional team and to facilitate and participate in education provision to meet those needs. To actively teach the nursing staff in the clinical area and evaluate their learning in conjunction with the senior nursing staff

3. To work collaboratively on educational strategies which support urology cancer services and service development

4. To develop and deliver education programmes on the ward in liaison with the multidisciplinary team/nursing staff.

5. Develop their knowledge, skills and expertise to a higher academic standard.

6. Be regarded as an expert resource working at a higher level in specialist areas.

RESEARCH / AUDIT ROLE

1. To have a good understanding of the value of quality research to improve patient care

2. To utilise up to date research/evidence to deliver patients care and to disseminate this knowledge to all members of the multi-professional team

3. To incorporate nationally agreed standards and guidelines into clinical practice.

4. To act as a resource for colleagues undertaking research.

5. To be aware of current research trials being conducted within own specialist area and to be able to inform patients about these

6. To audit the service on a regular basis so that patients care is continuously reviewed and improved, ensuring that documentation and record keeping is comprehensively maintained.

7. To contribute to clinical governance developments in line with the strategic direction of the trust

8. To share findings with others through publication and conference presentations.

LEADERSHIP AND CONSULTANCY:

1. To provide a leadership role model within the clinical setting demonstrating expert knowledge and high standards of clinical practice.

2. To engage with both internal and external groups which will lead to the further development of the nursing service and the Clinical Nurse Specialist role.

3. To monitor and evaluate ones own service implementing new initiatives as appropriate to improve patient care.

4. Contribute to the development of clinical services across the trust, through liaison with the Head of Nursing and other specialties.

5. To work with and alongside local and nation networks.

6. Work closely with other nurses to assist with their own professional development.

GENERAL
