Job summary

The successful candidate will offer high quality day to day care of patients and deliver a first class clinical service to include; outpatient/diagnostic clinics, day case theatre procedures, elective theatre activity, in patient care as required and support for on call work with acute urological admissions.

Main duties of the job

The appointed fellows will provide senior surgical input to emergency general surgery patients at all stages from initial triage and assessment to surgery and post-operative care. Duties include oncall at middle grade level with access to emergency theatre if the fellow is on call or assessment/admission of emergencies in A&E, surgical assessment unit/ SCDU when on normal duties.

The successful candidates will be able to rotate between Upper GI and Lower GI with dedicated MDTS. We are the 4th largest colorectal cancer resection unit in the UK.

The role will predominately include ward cover across the Department. There will also be potential for involvement with in-patient and day case operating sessions with general surgery and Urology at the Alexandra Hospital, Redditch. The successful candidate will also have exposure to outpatient clinics and outpatient diagnostic testing.

Our Department is recognised as holding Specialist MDT status for the management of patients with complex Urological cancer. We regularly receivecirca2500 two week-wait referrals to the Urology service with a conversion rate of 21%. Each year the Urology MDT reviews over 3000 case episodes.

Worcester City lying on the banks of the River Severn has the world famous cathedral, County Cricket Ground, Premiership Rugby and is home to the Royal Worcester Porcelain Factory. TRUST PROFILE

Worcestershire Acute Hospitals NHS Trust runs services from three main hospital sites: The Alexandra Hospital, Redditch; Kidderminster Hospital and Treatment Centre; and The Worcestershire Royal Hospital.

We also run some services and clinics at The Princess of Wales Hospital, Bromsgrove (outpatient clinics); Evesham Community Hospital (Burlingham Ward); Malvern Community Hospital (outpatient clinics); and Tenbury Community Hospital (outpatient clinics).

We have three main commissioners: Redditch and Bromsgrove Clinical Commissioning Group; South Worcestershire Clinical Commissioning Group; and Wyre Forest Clinical Commissioning Group.

ALEXANDRA HOSPITAL, REDDITCH

The Alexandra Hospital in Redditch opened in 1985. It serves a population of approximately 200,000 and has 360 beds. The hospital is the major centre for the countys urology service. The hospital has seven operating theatres, MRI and CT scanners and cancer unit status for breast, lung, urology, gynaecology and colorectal cancers.

There is a multi-disciplinary education centre with library, teaching and study areas.

Our plan for the Alexandra Hospital is to increase the amount of planned elective activity carried out at the hospital, especially in orthopaedics.

KIDDERMINSTER TREATMENT CENTRE

Kidderminster Hospital houses Kidderminster Treatment Centre which offers outstanding clinical facilities and patient accommodation for a wide range of daycase, short stay and inpatient procedures. The nurse-led minor injuries service is open 24 hours a day and treats more than 2,000 patients every month. It can deal with a wide variety of injuries including simple fractures, soft tissue injuries, lacerations, bites, burns and scaled. Other facilities at the Kidderminster site include a full range of outpatient clinics including outpatient cancer treatment in the Millbrook Suite MRI and CT scanners and a renal dialysis unit. There is also a modern education centre with seminar rooms, IT suite, library and break out areas.

Our plan for Kidderminster is to increase the number of daycase, short stay and inpatient procedures performed and for it to become an elective centre for the Trust.

WORCESTERSHIRE ROYAL HOSPITAL

Worcestershire Royal Hospital is the latest of the Trusts three sites. The main hospital was built under the private finance initiative (PFI) and opened in 2002. It provides specialist services for the whole of Worcestershire including stroke services and cardiac stenting. The hospital has nine operating theatres including four laminar theatres. It has a level 2 neonatal intensive care unit and a cardiac catheterisation laboratory. The 24/7 Primary Percutaneous Coronary Intervention (PPCI) service began in October 2013. It has 500 beds and serves a population of more than 550,000.

The countys brand new, state-of-the-art Oncology Centre was officially opened by HRH the Princess Royal in April 2015. Developed in partnership with University Hospitals Coventry and Warwickshire, the £25 million Worcestershire Oncology Centre will enable 95 per cent of radiotherapy to be delivered within Worcestershire. For patients, their families and their carers, who previously had to travel to Coventry, Cheltenham or Wolverhampton for radiotherapy treatment, cancer services will be more accessible than ever, saving an estimated one million miles of travel every year.

The Meadow Birth Centre has four birthing rooms and up to 1,000 women are expected to give birth in the unit every year. The brand new £500,000 birth centre is run by 14 experienced midwives and 7 maternity support workers, and opened in March 2015. The Centre offers a safe and comfortable environment for low risk women who have had no complications or medical problems during their pregnancy to give birth in. University of Worcester Vice Chancellor Professor David Green has praised the Meadow Birth Centre following a visit to celebrate the success of a joint working partnership.

Our plan for the Worcestershire Royal is for it to specialise in more complex services for patients from across the county, giving them access to services that they would previously have had to travel out of county for. We have recently opened our state-of-the-art Worcestershire Oncology Centre, which will provide radiotherapy services in county for the first time. Other developments include a dedicated Breast Unit - opening this year

MANAGEMENT STRUCTURE OF THE TRUST

Overall responsibility for the Trust rests with the Trust Board. Operationally the Trust is divided into six divisions Specialist Medicine, Urgent Care, Surgery, Womens and Childrens, Specialised Clinical Services Division and Corporate. Services are run on a countywide basis

Urology Services within the Trust

The Urology service runs a county-wide service which is based on upon a well-established hub and spoke model. 3 All elective inpatient surgery and emergency Urology is centred at the Alexandra hospital where we have Specialist Urology wards for elective and emergency patients. We have a fully equipped dedicated Urology theatre and our Robotic theatre is now fully operational. This houses the recently procured Intuitive, Da Vinci Xi System.

Outpatients, diagnostics and day-case activity is undertaken across all three Trust sites. At the Alexandra Hospital we have a standalone diagnostic unit (Urology Interventions Centre) which carries out:

- Flexible cystoscopy

- Urodynamics

- TWOC

- Intravesical therapy

- LATP

The service will be expanding to provide more focused acute Urology service integrated with the Urology Intervention Centre.

Each year our Urology Cancer MDT reviews over 3,000 cases and on average performs over 100 radical pelvic cancer procedures and over 100 renal tumour excisions. During the pandemic we continued to offer complex surgery including laparoscopy and open radical cancer excisions.

Our clinical outcomes have been submitted to HES/BAUS/GIRFT and NPCA for review and publication.

The Worcestershire Oncology Centre offers a full repertoire of treatment (including SABR) and support for patients diagnosed with Urological malignancy with the additional benefit of 24/7 out of hours support. In addition to general adult and paediatric Urology, Worcestershire offers a full range of specialist services including:

Laparoscopic renal surgery including partial nephrectomy and pyeloplasty.

Pelvic oncology including radical cystectomy, regional leaders in orthotopic bladder/complex reconstruction in urology, radical prostatectomy, in-county radiotherapy services.

Complex open renal and nephron sparing surgery including IVC tumour thrombus excision, renal artery embolization.

Robotic Surgery program started in October 2022.

Comprehensive stone surgery including percutaneous surgery, ESWL, flexible ureteroscopy, Swiss LithoVue, LaserClast and Auriga Laser.

Urodynamic and female Urology including fistula repair.

Andrology with specialist clinic support.

Holmium laser prostatectomy, REZUM, Bipolar TURP
