Job summary
An exciting opportunity has become available within the COPD Inreach Nursing Team for a 12 month fixed term or secondment to cover maternity leave. This role would suit a current Band 6 who wants to specialise in Respiratory or an experienced Band 5 who wants to progress.
The Inreach COPD Specialist Nurse team aim to review all patients admitted to hospital with an exacerbation of COPD within 24 hours of admission, complete medication and inhaler technique reviews, review and provide self management plans, disuss pulmonary rehabilitation and complete a discharge bundle.
You will also be involved in provided post discharge phone calls to patients, completing NRAP COPD Audit and working closely with the COPD Virtual Ward.
You will also be working closely with the community COPD Team, the specialist respiratory nurses at Worcester Royal and Alexandra Hospital, the Home Oxygen Team.
