Job summary

We are looking a friendly, motivated, committed individual to join our established Colorectal 2 week wait telephone Triage team Worcestershire Acute Hospitals NHS Trust for 15 hours a week. The post holder will provide telephone Triage assessment for patients referred on the 2 week wait referral pathway. As a Triage nurse you will be making clinical decisions regarding which patients are fit and appropriate to have investigations and referring patients for the most appropriate investigations.

You will need excellent communication and IT skills. Ongoing support will be available with the opportunity to develop personally and clinically whilst being involved in the development of an expanding cancer service. May require cross site working. Main duties of the job

As Colorectal Triage Nurse Specialist, the post holder will have an expert knowledge base, decision making skills and clinical competencies to provide a comprehensive telephone triage and clinical assessment service to patients referred by their general practitioner on the colorectal two week pathway

Work as part of the multi-disciplinary team communicating with GPs, colorectal consultants and cancer services etc.

Provide care and support for patients awaiting diagnostic tests.

The post holder will continue to develop the 2WW Colorectal Triage service, ensuring this encompasses the needs of the patients and is in line with Standards of good practice and cancer validation review recommendations.

The post holder will act as the key worker liaising with other Health Care Professionals to provide the best possible continuing care for patients. He/she will co-ordinate care through the patient journey from diagnosis to discharge.

  • RGN with current registration
  • Evidence of Continuing Professional Development
  • NMC recognised teaching and assessment qualification
  • First level degree or working towards

Desirable

  • Advanced Communication Skills Course
