We are delighted to offer an exciting new
opportunity for a dynamic Tissue Viability Health Care Assistant to join Worcestershire
Acute Hospitals NHS Trust Tissue Viability Team .
The post holder will be instrumental in supporting
the team in delivering The Trusts Quality Improvement Strategy: The Patient
Safety Plan which documents the ambition to reduce the incidence of Hospital
Acquired Pressure Ulceration.

To participate and support in the provision of a
specialist Trust wide nursing service, working closely with the
multidisciplinary teams. The post holder will support and assist the Tissue
Viability Team with complex patients , projects and maintaining clinical
databases .
The service covers 3 hospital sites and service
delivery is currently working over 5 days between the hrs. of 08.00 to 16.00
hrs. so flexibility is essential. Current base is Worcester Royal Hospital

Main duties of the job
To assist the Tissue Viability Team with projects and complex

To be an active team member of the Tissue Viability Team
assisting with Tissue Viability educational programes .
Supporting the TV team with the day-to-day running of the Tissue

Risk Assessment: This post involves frequent contact with patients some with infectious diseases and
conditions. This post involves manual handling of equipment, daily use of VDU screen and exposure to
blood, body fluids, wound exudate and discarded sharps/clinical waste.
Job Purpose:To participate in the provision of a specialist Trust wide nursing service. The post holder will
support and assist the Tissue Viability Team working closely with the multidisciplinary team .
They must also possess knowledge and skills in clinical risk assessment and evaluation, pressure ulcer
prevention strategies to assist the Tissue Vibility Nurses in training across organisational and professional
boundaries. To actively support the Trust Quality Improvement Plan, Safety Plan in order to meet Trust
Trajectory regarding Pressure Ulcer incidence under the Safer Care framework, by supporting the Tissue
Viability Team undertaking data collection, audit and surveillance of pressure ulcers under the guidance of
the Lead Tissue Viability Nurse Specialists.
To assist the Tissue Viability Team with projects and complex patients.

Key Duties:
1 CLINICAL PRACTICE
To actively support the Trust Quality Improvement Plan, Safety Plan in order to meet Trust
Trajectory regarding Pressure Ulcer incidence under the Safer Care framework
Surveillance of All Hospital Aquired Pressure Ulcers a, deep tissue injury pressure damage and
Incontinence /Non Incontinence Associated Dermatitis under the direction of the tissue viability
nurses.
Assisting the tissue viability nurses with complex wound management.
To contribute to a person centred care approach, maintaining good communication with all
members of the multidisciplinary team.
Assist the tissue viability nurses to work in partnership with all members of the health care team to
ensure consistently high standards of pressure ulcer prevention.
To demonstrate competency in the role, be willing to attend relevant courses, according to identified
training needs
Contribute effectively as a member of the multi-professional team, building appropriate
relationships.
To update the patient records and complete and update nursing assessments as required
To receive and make telephone calls, which could often be of a highly sensitive nature, providing a
first point of contact to members of staff, the public, patients and their relatives and other agencies
in the absence of the Tissue Viability Team. This will include assessing calls and either responding
/ advising as appropriate, referring to the Tissue Viability Team Nurses or taking accurate detailed
messages if necessary.
To assist the Tissue Viability Team with correspondence including opening, prioritising and
processing as required.
To be responsible for Negative Pressure unit log / stock / stationery control. Ordering of equipment
and specialised clinical items for use throughout the hospital at times of specific need, followed by
appropriate financial cross-charging.
To assist in times of need by entering data onto the equipment store database.
Assit and participate Tissue Viability Audits .
2 EDUCATION , TRAINING AND DEVELOPMENT
To assist with the development, delivery and evaluation of Tissue Viability related educational
programmes
To educate and work in partnership with patients and carers on various aspects of wound
management and pressure ulcer prevention
Communicates effectively by giving and receiving information so that appropriate action can be
taken.
Act as a centre point for supporting other healthcare assistants
Act as a resource to nurses, other professionals and students on placement in relation to Tissue
Viability.
At all times ensure that own actions support and promote equality, diversity and the rights of

3 PRACTICE AND SERVICE DEVELOPMENT
Be vigilant in ensuring all care being delivered adheres to policies and procedures relating to
pressure ulcer prevention and support other healthcare workers in doing the same. Refer noncompliance with policies to the nurse in charge and the tissue viability nurses
Maintain own competence by continuously developing own skills and knowledge in accordance with
agreed personal development plan.
Participate in regular 1-1 meetings with line manager. The post holder will have an appraisal or PDR
after two months with their Line Manager and this process will include the development of a
Personal Development Plan.
Maintain a participatory role within the team; new ways of working, new ideas, improvements to be
discussed as a team
In the discharge of their duties the post holder may often be in a position of or have access to
confidential/personal information and must not disclose or discuss such information outside their
place of work, or within their place of work except in the proper discharge of their duties. Failure to
observe confidentiality may result in disciplinary action.
To support and co-ordinate with various aspects of the Tissue Viability Team audit programmes.
This requires competence and experience of Microsoft Windows computer programmes and will
require participation in planning audit programmes and collection of data .
To assist with arranging /supporting the TV Team for training to ensure appropriate support services
available, supplies of teaching materials and handouts as required.
Undertake general photocopying and distribution of papers.
To maintain a high degree of confidentiality at all times, no matter how difficult, with regard to
information concerning members of staff and patients, ensuring that all work practices comply with
the Data Protection Act.
Prepare information in the desired format i.e. report, spreadsheets, charts etc.
Show a flexible approach in a changing, demanding environment which has an unpredictable
workload.
Communicate effectively and courteously when receiving complex or sensitive information that
comes into the department, where tact and re-assurance is necessary.
