Job summary

We are delighted to offer an exciting new opportunity for a dynamic Tissue Viability Health Care Assistant to join Worcestershire Acute Hospitals NHS Trust Tissue Viability Team .

The post holder will be instrumental in supporting the team in delivering The Trusts Quality Improvement Strategy: The Patient Safety Plan which documents the ambition to reduce the incidence of Hospital Acquired Pressure Ulceration.

To participate and support in the provision of a specialist Trust wide nursing service, working closely with the multidisciplinary teams. The post holder will support and assist the Tissue Viability Team with complex patients , projects and maintaining clinical databases .

The service covers 3 hospital sites and service delivery is currently working over 5 days between the hrs. of 08.00 to 16.00 hrs. so flexibility is essential. Current base is Worcester Royal Hospital

Main duties of the job

To assist the Tissue Viability Team with projects and complex patients.

To be an active team member of the Tissue Viability Team assisting with Tissue Viability educational programes .

Supporting the TV team with the day-to-day running of the Tissue Viability Service. Answer phone calls & message taking, input and maintain clinical data bases.

Assisting the tissue viability nurses with complex wound management. To contribute to a person centred care approach, maintaining good communication with all members of the multidisciplinary team. Assist the tissue viability nurses to work in partnership with all members of the health care team to ensure consistently high standards of pressure ulcer prevention. To demonstrate competency in the role, be willing to attend relevant courses, according to identified training needs Contribute effectively as a member of the multi-professional team, building appropriate relationships. To update the patient records and complete and update nursing assessments as required To receive and make telephone calls, which could often be of a highly sensitive nature, providing a first point of contact to members of staff, the public, patients and their relatives and other agencies in the absence of the Tissue Viability Team. This will include assessing calls and either responding / advising as appropriate, referring to the Tissue Viability Team Nurses or taking accurate detailed messages if necessary.
