COMMUNITY HEART FAILURE NURSE - BAND 7

34 HRS PER WEEK

This is an exciting opportunity for an enthusiastic, confident, well-motivated, autonomous RGN, who works well in a team, as well as on their own, to join our team of Heart Failure Specialist Nurses.

The ideal candidate will be highly motivated with experience in chronic heart failure and experience working at band 6/7. Excellent interpersonal skills together with the ability to work autonomously and communicate effectively across a multi-professional team to initiate and facilitate the management of your own caseload of patients are an essential part of the role.

If you have a desire to work in a proactive, friendly team and have experience in delivering care to patients diagnosed with heart failure we are keen to receive your application.

Main duties of the job

Skills for clinical and risk assessment, cardiovascular examination, and health education of the patient with chronic heart failure are required for this rewarding area of work. Evidence of providing education is highly desirable.

Job description

Job responsibilities

To assess patients in their home environment and/or a clinic environment and plan for their future needs in accordance with the service guidelines.

To demonstrate a high level of expertise in the field of community heart failure services provision through a comprehensive assessment of heart failure patients and the application of this knowledge to advise on the development of services across organisational boundaries.

To identify patients for ambulatory diuretics and liaise with Secondary Care and to support and participate in Rapid Access Heart Failure clinics alongside Heart Failure Consultants and the Acute Team.

Management of individual caseload assuming ability to work autonomously within scope of practice:

Clinical assessment.

Drug titration and prescribing.

To select, apply and advise on profession specific assessment and treatment strategies for heart failure patients with complex presentations at any stage of the heart failure syndrome.

Ensure clients receive comprehensive education regarding diagnosis.

Optimise referrals and patient pathways and avoid hospital admissions by developing and participating in the running of clinics/services such as rapid access Heart failure Clinic.

To provide support for patients and their families, particularly where the diagnosis is recent, those in need of palliative care and those experiencing deterioration in their quality of life with regard to physical, psychological, social and spiritual elements of care.

Support the service leads with the co-ordination of the overall Heart Failure Care Pathway.

Take every opportunity to promote the health of patients and initiate health education strategies as necessary, such as smoking cessation and cardiac rehab.

To work with the cardiac rehabilitation team in order to provide a service for heart failure patients where appropriate.

To identify patients requiring input from specialist palliative care services, seeking advice from and liaising with these services.

Provide emergency care within the scope of practice and be familiar with cardiac resuscitation guidelines.

  • Experience of audit and evaluation.

  • Knowledge of current Local National and International Heart Failure guidelines.
  • IT literate.

  • BLS.
  • Good verbal, written and communication skills.
