Job summary

An exciting opportunity has arisen to join our Haematology and Oncology Directorate. We are looking for individuals who are passionate about cancer care and would like the opportunity to work alongside our fantastic nursing team within our Systemic Anti-Cancer Treatment (SACT- Chemotherapy) units at Worcestershire Royal Hospital.

We are keen to hear from individuals who have the necessary drive to support this change in service delivery. The SACT Co-ordinator role will support the clinical teams in ensuring all patients move smoothly through the SACT pathway, acting as a co-ordinator of care and ensuring the management of patients receiving SACT on the units is efficient and effective, taking into account the individual needs of the patients and the requirements of the service. Your role is vital to streamline the patient journey and contribute to the improvement of both quality and experience.

We are looking for confident, enthusiastic and conscientious team players with excellent time-management and organizational skills who can work autonomously and well under pressure. The post holder will be required to have excellent communication skills and an ability to deal with complex situations with service users and clinicians.

Main duties of the job

Working under the direct/indirect supervision of and in support of the registered nurse, assist in the smooth and effective running of the Systemic Anti-Cancer Treatmant (SACT) Units

To act as a co-ordinator of care and ensure the management of patients receiving SACT on the units is efficient and effective, taking into account the individual needs of the patients and the requirements of the service.

Facilitate the (SACT) patient pathway by co-ordinating and proactively tracking the timeliness of patient treatments within the National guidelines ensuring all pre-treatment investigations are carried out prior to SACT.

Work through patient treatment lists with the registered nurse checking everything required is in place prior to commencing SACT, liaising with relevant professionals / patients where necessary

Job responsibilities

The key elements of the role are:

To work under the direct/indirect supervision of and in support of the registered nurse, in the smooth and effective running of the Systemic Anti-Cancer Treatmant (SACT) Units

To act as a co-ordinator of care and ensure the management of patients receiving SACT on the units is efficient and effective, taking into account the individual needs of the patients and the requirements of the service.

Excellent organisational and communication skills together with high levels of accuracy are also a key requirement for this post.

Work with nursing and scheduling staff to ensure all patients have appropriate appointments booked, and that pre-treatment investigations and procedures have been undertaken with results available before treatment.

Ensure prescriptions have been completed and results of appropriate investigations have been recorded prior to commencing SACT

Triage incoming calls and initiate appropriate response in accordance to assessment tools, protocols and individual pathways, liaising with the SACT nurses and CNS as appropriate.

Make appropriate telephone calls to patients when required, Informing patients of relevant appointments, providing basic telephone advice and refer on or signpost to other sources of support.

Ensure any investigations and diagnostics are booked promptly eg, blood tests, ECHOs and PICC line insertions, and district nurse referrals completed where necessary

Co-ordinate patient documentation, including investigation results, prior to and during their attendance at the SACT treatment unit

Proactively identify emerging issues that may cause delays at any point in patient pathways and take action to improve administration or any other aspect of the SACT pathway. Escalate to the nurse-in-charge where necessary

To have knowledge and understanding of all of the cancer pathways and have an active role in redesigning pathways and service improvement, particularly in the analysis of existing pathways

To proactively monitor patients progress along their pathway, highlighting problems to the SACT unit managers / Lead Chemotherapy Nurse and escalating where necessary according to local protocols.

Negotiate with and instruct relevant personnel, departments of clinic bookings/diagnostic tests/admissions that need to be arranged or bought forward to ensure prompt treatment.

To understand SACT service provision for cancer patients within the ICS, the steps along the patient pathway and the associated flows of information. This will include referral processes, diagnostic and treatment options and involves using the Trusts computer systems (CLIP, ICE, WINPATH, CRIS, PACs, OASIS, Bluespier, MOSAIQ, System One, LCMS).

Undertake general administrative tasks according to unit requirements

General Duties:

To communicate effectively and professionally, giving reassurance, information or advice appropriately.

Assist in the training of new SACT Co-ordinators.

Attend and participate in team meetings.

Communicate fully with the wider cancer team including both clinical and non-clinical members, at all levels internal and external to the Trust to ensure the best possible outcome for SACT patients.

Participate in the Trusts Performance and Development Review and to undertake any identified training and development related to the post.

Undertake statutory and mandatory training as deemed appropriate by the Trust.

Develop and maintain effective working relationships with colleagues.

Adhere to all Trust policies and procedures. Ensuring confidentiality at all times.

Undertake such other tasks as requested by the Directorate Support Manager in accordance with the grade and nature of the post.

Maintain a smart and professional appearance at all times.

Maintain a professional manner including practising respectful office etiquette at all times.

The post will be regularly reviewed and additional duties may be assigned as necessary in direct response to the changing Cancer and SACT agenda.
