Job responsibilities
The post holder will be a member of the Clinical Site Team. The required functions of this post will be:
- To be responsible for maintaining 4 hr emergency access target & 18-week surgical elective target.
- To be set a target by the Clinical Site Matron/Clinical Site Manager on a daily basis for predicted discharges.
- To ensure emergency admissions are placed in the appropriate clinical areas.
- To ensure effective use of the Medical Assessment Unit, Surgical Assessment Unit, both medical and surgical SDEC & A&E Observation Unit & to prioritise emptying these units proactively throughout the day.
Key Duties:
Work as a member of the Clinical Site team to provide an effective and pro-active clinical site management service for all impatient admissions.
Liaise effectively with Accident & Emergency staff regarding emergency admissions to ensure that the 4 hr emergency access target is adhered to.
Allocate beds for emergency admissions as required, taking into account, in discussion with clinical teams, clinical appropriateness and priority.
Work with surgical ward staff to ensure that elective admissions are accommodated and escalate appropriately when capacity is not available.
Ensure that patients admitted to the Medical Assessment Unit are appropriately transferred to wards with the minimum delay.
Ensure patients assessed on MAU, SAU are appropriately placed as soon as a decision to admit has been made.
Ensure patients in A&E Observation Unit who require further treatment & admission are prioritised to a ward bed in order to keep A&E patients flowing through the unit.
Work to a daily agreed target of potential discharges, identifying where most discharges will be from & actively participating in facilitating these discharges, simple & complex.
Where necessary contact consultants in charge of patients care to review their patients & facilitate a discharge plan.
Ensure Clinical teams / ward staff are documenting estimated discharge dates & adhering to them.
Ensure ADT whiteboards are kept up to date on each ward during the day to enable the implementation of a real time bed state.
Liaise with other Trusts to facilitate the transfer and repatriation of patients.
Manage non-urgent medical admissions.
Understand the importance of accurate information to enable self and on call managers to make appropriate decisions & potential issues that may arise if not the case.
Work closely with A&E/MAU Co-ordinators to ensure a seamless, 24-hour bed management service.
Monitor outlier levels and repatriate all outlying patients as appropriate, taking into account continuity of care and liaising with speciality bleep holders regarding clinical appropriateness.
To have knowledge of the Critical Care transfer out protocol and be aware of Critical care capacity and staffing matrix to enable safe transfers of level 2-3 patients.
Escalate any professional concerns to appropriate matron / manager.
Ensure proactive planning and identification of patients for transfer to Aconbury unit.
Ensure that the discharge lounge is used and deployed effectively & patient transport services are being effectively organised & planned.
Contribute to ensuring that data entry practices relating to admissions, transfers and discharges are consistent with both local policy and national data definitions.
Ensure timely and accurate completion of data relating to bed utilisation.
Ensure that escalation is actioned when appropriate in line with Escalation Policy and play a key role in working effectively with partnership organisations.
To be able to deal with a complaint from a patient or relative on site or on the telephone in a Professional & timely manner.
To be responsible for ensuring that site information for the Countywide Bed State is accurate.
Act as Incident Officer for fire incidents / alarms.
Attend all infection control out break meetings
Take Lead role in Major incidents
Chair bed meetings and take actions in the absence of Site lead at escalation levels 1 & 2
When extra bed capacity is opened maintain an overview of the area ensuring that communication with key stakeholders is robust and staffing is adequate. Ensure that extra capacity is closed at the earliest opportunity.
When required, undertake role of Night Co-ordinator to include site management and incident manager, including initial co-ordination of MAJAX.
Participate in ensuring that the capacity management rota provides appropriate and effective cover.
Monitor Ambulance Turn round Times and escalate to senior Management team
Escalate any delayed discharges
