CARDIAC REHAB NURSE SPECIALIST
ALEXANDRA HOSPITAL, REDDITCH
BAND 6
22.5 HRS

This is an exciting opportunity for
an enthusiastic, confident, well-motivated, autonomous RGN, who works well in a
team, as well as on their own, to join our team of Cardiac Rehabilitation Nurses
and will be based at the Alexandra Hospital.
The position would suit someone
with previous sound cardiac or related knowledge who is currently a senior Band
5 or above.
Our ideal applicant will have
excellent interpersonal skills and leadership qualities, you will have drive,
vision and initiative to motivate others and develop this service. Excellent
decision making skills and evidence of continuous professional development are
essential and you must also have experience of working in both cardiology and a
multi-professional environment.

Main duties of the job
You will be supported to facilitate the management of your own caseload of patients and support the phase 3 fitness classes at the Alexandra Hospital and Evesham gym as an essential part of the role. You will also need to have excellent written skills, be computer literate and hold a clean driving licence. This post is Countywide and the successful applicant may be required to support services on all 3 sites.

Job responsibilities
The post holder will work closely with members of the multi-disciplinary team to provide a countywide cardiac rehabilitation service that delivers education, advice and tailoredexercise programmes to patients with acute and chronic coronary syndromes, post surgical patients and heart failure patients.
- To give expert advice on management of cardiac patients including medication and further treatment options.
- To manage a caseload of patients.
- To act as a clinical advisor on cardiac rehabilitation to all members of the multidisciplinary team.
- Participate in updating CHD guidelines, protocols in line with clinical governance guidelines.
- Promote rehabilitation patient autonomy as a focus of care.
- Make contact with patients during acute episode and perform risk factor assessments.
- Review patient notes to ensure all MI/Heart Surgery/ACS patients are seen for advice/support, and relevant investigations are requested in line with trust guidelines and cardiology teams.
- To participate in cardiac rehab Phases I III as per BACPR guidelines
- To act as the patients advocate by discussing treatment approaches, writing letters to consultants/GPs to maintain a good quality of life for patients while reducing risk factors.
- To see inpatients/outpatients for pre surgery/investigations advice support
- To provide a support line for rehabilitation/RACPC/Angiography patients and relatives to reduce anxiety for all clients involved/beneficiary of cardiac rehab service.
- To work closely with the Cardiac Rehabilitation fitness instructor maintaining a team approach to high standards of care.
