Job summary

An exciting opportunity has arisen within the Medicine Directorate for a Data & Audit Officer working within the Stroke team based at Worcestershire Royal Hospital, supporting with SSNAP targets. This role is pivotal to the collection of a wide range of data across the patients journey following a stroke or transient ischemic attack. Data accuracy is crucial to the monitoring and reporting of service quality, underpinning service planning as necessary.

Main duties of the job

Design of the data collection process working with clinical teams in the designing of processes to enable the collection of data in line with network agreed data fields

To support clinical teams in the collection of data for the Sentinel Audit

Being conversant with SSNAP fields and definitions required for the collection of real time audit data

Monitor data and notify designated lead if required targets are not being achieved and assisting with the development of a responsive action plan

To ensure timely generation running of reports to enable all data to be validated by lead clinician before submissions to formal data systems

Support the development of audit projects, including the design of data collection forms, advice on suitable methodologies, design of databases and audit presentations.

Facilitate, with support of colleagues, the dissemination of audit findings and implementation of best practice by working with clinicians to produce final reports with recommendations for future audit and assisting with presentations if appropriate.

Ensure that all allocated projects are undertaken in an efficient and timely manner, meeting agreed deadlines quality standards

Job description

Job responsibilities

1. To be conversant with the clinical pathway from the onset of symptoms to discharge from acute care.

2. To work with clinical teams in the designing of processes to enable the collection of data in line with network agreed data fields/sets.

3. To ensure conversant with network/commissioning specifications to ensure the adaptation of design of data collection systems to collect commissioner/performance data.

4. To ensure the development of an audit proforma that can be used to collect data through the patient journey.

5. To pull notes as required to fill in the gaps that have not been completed on the audit proforma.

6. To work with colleagues in provider services to ensure the collection of data/additions to the data set as required by commissioners.

National Datasets

7. To support clinical teams in the collection of data for the Sentinel Audit, ensuring the collection of medical records/inputting data as required.

8. Be fully conversant with the SINAP fields and definitions required for the collection of real time audit data.

Performance Monitoring

9. Notify designated lead when data indicates that the required targets are not being achieved and assist with the development of a responsive action plan.

10. To ensure timely generation running of reports to enable all data is validated by lead clinician before submissions to formal data systems.

11. To be conversant with the submission dates for vital signs indicators, ensuring the data is submitted to the performance team for formal reporting to enable timely reporting.

12. To provide ad hoc summaries on progress as requested/ensuring the appropriate consent has been gained prior to release.

13. To work with provider services to distribute evaluation forms to patients, service users and carers to obtain feedback on the quality of service.

14. To produce some simple analysis on feedback, providing the first draft for analysis by operational management.

15. To support the completion of action plans to address patient issues / comments / recommendations.

Audit

16. Assist with other audit activities related to the National Stroke Strategy and where necessary undertake the required activity.

17. Assist clinical staff in the identification of audit priorities and advise on the viability of audit topics in compliance with Trust objectives and regional / National priorities.

18. To co-ordinate, in liaison with the lead clinician, matrons and directorate manager, the organisation, planning and delivery of clinical audit activities as required by National policy documents/ local internal policy.

19. Assist clinical staff in the identification of audit priorities and advise on the viability of audit topics in compliance with Trust objectives and regional/National priorities.

20. Assist clinical staff in the review of pertinent literature, evidence and review articles to support the development of an audit project.

21. Support the development of audit projects, including the design of data collection forms, advice on suitable methodologies, design of databases and audit presentations.

Essential

  • Educated to GCSE Grade C, including English and Maths
  • NVQ3 or equivalent qualification in relevant subject.
  • RSA typing Stage 1

Experience

  • Previous experience of working in an administrative service and / or customer care environment, ideally in an acute hospital setting.
  • Experience with Data Collection.

Desirable

  • Familiarity with medical terminology.
  • Experience with SSNAP / SSNAP data.
