Job summary
The post holder will be a key worker in the
Vascular MDT and will utilise advanced nurse practitioner nursing skills, &
expert clinical knowledge in the diagnosis and management of patients with
vascular disease and its associated conditions. The post holder will provide a
high level of expert nursing practice and advice and support for patients with vascular
disease and related conditions in all care settings throughout the Trust.
Main duties of the job
Provides expert, specialist assessment of patients with vascular disease and its related conditions
following referral to the vascular service. Realistically improving the holistic
care of patients within the service.Liaison with colleagues to
ensure continuity of care.Establish professional relationships with the patient and acts as the patient advocates within the MDT
setting.Active participation in the weekly MDT
meeting.Initiates and participates in highly
sensitive and complex communication issues with patients, carers,
nursing/medical team.Attends clinic as per timetable
providing specialist advice and support together with autonomous
practice.Review of defined caseload of patients with
vascular conditions during their admission. Provides
specialist telephone advice to patients and healthcare professionals in both
secondary and primary care with regard to management of vascular conditions.Develops specialist nurse led care, where
appropriate, in line with national guidelines.Participates in designated research and audit
projects, completing all required documentation, reporting to and liaising with
the research team.Working within the ward environment,
attending Morning Ward Round and facilitating patient care and safe discharge
,utilising advanced nursing practice skills & knowledge.

vascular disease and its associated conditions. The post holder will provide a
high level of expert nursing practice and advice and support for patients with vascular
disease and related conditions in all care settings throughout the Trust. The post holder will undertaken advanced structured patient assessment utilising diagnostic skills to establish a differential diagnosis of patient s vascular disease within the clinical pathway.
The post holder will request and interpret
appropriate investigations for patients with vascular disease and its related conditions,
triaging the urgency of need for intervention.The post holder will demonstrate a high level
of professional and autonomous nursing practice. Using comprehensive and advanced knowledge and skills the post holder will plan care and implement specialist treatments for patients with vascular conditions. The post holder will assess the efficacy of treatment/intervention in the ongoing episode of patient care.
The post holder will act as expert specialist
nursing resource for patients with vascular conditions collaborating with all
members of the multi-professional teams within both the Acute and Primary care
settings to facilitate a collaborative and cohesive approach to patient care.The post holder will actively participate in service
developments for patients with vascular disease and its associated conditions
in association with the Consultant Nurse for Vascular Surgery; ensuring this
encompasses the needs of the patients and is in line with the standards of
evidence based practice.The post holder will act as the key worker
liaising with other Healthcare Professionals to optimise continuing care for
patients.The post holder will provide in-house training
to be able to optimise care across both OPD and ward settings. Developing
In-House training and education for Trust staff to facilitate the optimal
management for patients with vascular disease and conditions including leg
ulceration and complex wounds within vascular services.
