Job responsibilities

Work with EPUT Community Teams in particular Older Adults Mental Health, Dementia Localities, DIST, Primary Care Networks (PCNs), Local Authorities (LAs) and Voluntary / Third sector services across Southend, Castle Point & Rochford to support vulnerable and ageing carers (over 65)

You would identify and support opportunities for Carers Health and Wellbeing Checks to

• Ensure early identification of respiratory concerns.
• Ensure carers uptake of flu, pneumonia and other vaccinations to prevent hospital admissions and ability to maintain caring role
• Ensure opportunities to discuss lifestyle choices such as smoking, diet, inactivity, that may contribute to increased risk of cancer
• Undertake or refer for BP, Pulse and other physiological monitoring
• To identify rising carer burden in older carers in deprived areas and take action before carer breakdown
• Identify rising carer burden and support those carers who are disadvantaged either by literacy deficits or digitally to access all available support to build understanding and resilience for the future
• Support those to access help if they are facing fuel and/or income poverty, digital and literacy exclusion and deprivation
• Using Carer Health checks to identify lifestyle choices such as smoking, diet, inactivity that may contribute to increased risk of cancer.
• Identify those at risk of being isolated and lonely and facilitate support to minimise this
• Encourage at older adults with mental illness or concerns to seek help from OPCMHT. Identify and support to access NHS Talking Therapies services for common mental health issues such as anxiety & depression
• To educate carers on available services and opportunities to access social and therapeutic activities for carers supporting engagement to reduce isolation and loneliness
• Support carers with understanding the importance of contingency planning

The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our populations health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a one workforce cross system approach to the onboarding, induction, education and experience of Healthcare Assistants.

To support this one workforce model the successful candidate for this post will be required to attend our5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process.

Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience.

This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).

Person Specification

Education and Qualifications

Essential

• Student Associate Practitioners (band 3), be prepared to undertake study or have relevant experience.
• Full UK Drivers Licence & Access to own Car

Desirable

• Relevant qualification or experience in Community Development / Education, Health or Social Care

Experience and Skills

Essential

• A good understanding of the needs of people with dementia, who are frail or in the last 12 months of life and their carers
• Treating patients & their carers / family with respect and dignity
• Methodical organised approach to work with good attention to details

Desirable

• A good understanding of older adult mental health issues and its treatment/management
• Knowledge of Trust ITT Systems and Software Packages, including Mobius and SystmOne
