Job summary

We are seeking a dedicated and compassionate Band 3 Support Worker to join our North East Essex Dementia Service.

Our team is committed to delivering a responsive, flexible, and evidence-based specialist service for individuals undergoing assessment for memory problems and for those diagnosed with dementia. You will play a key role in supporting people with complex presentations and needs, helping them access timely assessments and appropriate care.

About the Service

Based at Kingswood Centre, the North East Essex Dementia Service provides assessment, diagnosis, treatment, and ongoing support for individuals experiencing memory problems linked to neurodegenerative conditions. We work in close partnership with third-sector organisations, particularly the Alzheimer's Society, to ensure that people using our service, and their carers, receive tailored support and effective onward signposting.

Your Role

o Support efforts to reduce unnecessary hospital admissions to both general and mental health units

Main duties of the job

Assisting qualified nursing staff in providing care and support of people with a diagnosis or suspected diagnosis of dementia and their carers, within the acute trust, mental health trust, community settings and/or patient's own home. Providing person centred care and working towards their goals within their care plans.

The role is to support admission avoidance, including working directly with Primary Care services, social care services, third sector organisations, non-statutory care providers and local GPs. The work will also include working directly within the Acute Trust. And inpatient mental health services to support timely discharge and post discharge support.

The post holder is responsible for reporting regularly to the Team Lead or other qualified staff, working both under supervision and on their own, reporting any relevant changes in the patient's condition and environment.

The post holder is responsible for undertaking assigned tasks that involve providing direct patient care in the community. Patients, relatives, carers and other staff will find the post holder flexible, approachable and with a helpful disposition.

The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our populations health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a one workforce cross system approach to the onboarding, induction, education and experience of Healthcare Assistants.

To support this one workforce model the successful candidate for this post will be required to attend our5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process.

Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience.

Further information will be provided post interview.

This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).
