Community Mental Health Practitioner

Job summary

The post holder will be based at C&E Centre, Chelmsford and hold a caseload of service users with moderate to severe mental health problems and work within the agreed model of care for the service. The post holder will work as an accountable practitioner under the preceptorship of senior staff. This role will involve assessment, planning, implementation and evaluation of the care provided and effective liaison with all agencies involved with client or service user.

The post holder will be supported by a multidisciplinary team comprising of nurses, doctors, psychologists, peers, OT's, employment specialists etc
