Job summary
The post holder is accountable to the Operational Services Manager, and will be clinically supervised by the Dementia & Older People's Community Mental Health Team Manager, but is required to undertake the duties of the post without direct supervision.
The post holder is responsible for the assessment of care needs and to assist with the development, implementation and evaluation of programmes of care for individuals with dementia and whose identified needs are commensurate with the skills required of this grade. The post holder will offer advice, support and signposting to individuals and their carers and work very closely with the other Dementia and Older People's Mental Health Teams, Dementia Navigators, integrated community services, primary care, third sector organisations and others involved in the individual's care and carer support.
The post holder will offer assessment and support through the diagnostic journey of the individual and their carers/family.
The post holder will support GPs in ensuring reviews are completed for their practice register and be the contact point for primary care for people with dementia and their families.
The post holder will ensure that an integrated service is provided to the patient, liaising with other services as required
Main duties of the job
1 The post holder will be expected to act as an allocated liaison senior nurse, undertaking assessments and assisting with the planning, implementation and evaluation of individualised person centred care plans for individuals with diagnosed and or suspected dementia, as required.
2. The post holder will have close links in with the other teams in the Dementia & Older People's Community Mental Health Service.
3. The post holder will facilitate diagnosis by assessment and use off the appropriate individualised pathway.
4. The post holder will be required to fast track to dementia diagnosis, via suitable qualification and support.
5. The post holder will work with the community pharmacist if medication reviews and or prescribing are required.
6. The post holder will be expected to offer expert and appropriate prescribing advice on the treatment of dementia and related psychological and behavioural aspects, and to act as an independent prescriber in the role if they hold the relevant qualification.
7. Ensure effective lines of communication and good networking exists, both with EPUT and with other Community, Primary and Social Care Services.
8. Disseminate specialist knowledge across the range of work areas and practices underpinned by sound theoretical knowledge.

ROLE SUMMARY
The post holder will be responsible for reviewing the care needs of patients with dementia (QOF) in primary care .The post holder will provide advice to staff/professionals on appropriate care, interventions and management on anything dementia related and signpost individuals to other services as required . It is expected that the post holder will at all times uphold and focus on the Recovery model, person centred care and be mindful of all physical health needs and social inclusion for people with dementia and their carers.
A key aspect of the role will diagnosis assessment and advice on specialist medication and undertaking medication reviews as required hence a recognised Independent Nurse Prescriber qualification is a key expectation of the role. It is also anticipated that the post holder will hold (or be willing to undergo) the Identification of Dementia Syndrome Status.
The post holder will have close links with Primary Care colleagues within their locality to ensure dementia review care plans are in place and assisting primary care colleagues with QOF register dementia reviews.
The post holder will have close links in with the DIST team.
The post holder will provide advice and triage to clients with a Dementia diagnosis and work with the GP to sign post to appropriate services when urgent care interventions are required eg DIST
The post holder will work with the community pharmacist if medication reviews and or prescribing are required if they are Nurse Prescribers
Ensure effective lines of communication and good networking exists, both with EPUT and with other Community, Primary and Social Care Services.
Disseminate specialist knowledge across the range of work areas and practices underpinned by sound theoretical knowledge.
To participate when required with practice MDTs.
Responsible for influencing policy developments within their catchment area and participate in steering groups, which influences policy within EPUT and Primary Care.
Actively and constructively contribute in Team Meetings, initiating and discussing new methods/models of working
Participate in the introduction of research or trials with the area.
Inform the Team Manager if they are concerned with regards to any breaches of professional conduct.
Have a sound knowledge/understanding of the legislation as part of the role of the Registered Mental Health Nurse, including Mental Health Act 1983, Misuse of Drugs Act 1971, Access to Information Act 1991, Health and Safety at Work Act 1974 and CPA. NICE Dementia Guidelines and National Dementia Strategy.
In association with Anglia Ruskin University, University of Essex and Trust Advisory Nursing Group (T.A.N.G.), ensure the learning environment for all nurses in training is appropriate and that teaching areas comply with all N.M.C. Regulations.
Participate and take professional responsibility/accountability and ensure that accurate, timely records are maintained in line with the Trust Policy.
Ensure that clinical practice is evidence-based wherever possible and participate in research and audit.
Use research findings when designing care programmes and to develop and awareness of the significance of nursing practice.
Keep updated in all legislative government initiatives.
Following any serious incident ensure that appropriate documentation is fully completed in line with Trust Policy and Procedures CP2, CPG2, CP3 CPG3 and forwarded to the Team Manager.
Have an awareness of the complaints procedure as EPUT policy and attempt to resolve any issues by local resolution in the first instance and assist with the investigation of complaints as required.
Be responsible for updating own knowledge of contemporary psychiatry and a commitment to continuing own personal and career development, in line with Trust Appraisal and Development Policy
Attend training as provided and be responsible for keeping self up to date at all times with mandatory training.
