Clinical Lead End Of Life

Job summary

The post holder will work alongside the Clinical Lead for end of life care on Trust wide initiatives to promote and improve the standards of end of life care across Essex Partnership University NHS Foundation Trust. Within this development role, they will gain Highly Specialist skills to support end of life care across all services within each area of the organisation.

The post holder will work closely under the guidance of the Clinical Lead with all partnership organisations to support best practices including care co-ordination and service development. They will be responsible for promoting a shared culture, vision and values to deliver corporate and team objectives, while supporting the measurement system to monitor the quality of end of life services. They will support the implementation of competencies for professionals involved within the delivery of end of life care and will support multi-professional education to assist with embedding and sustaining the policies and guidelines associated with end of life.

They will be responsible for ensuring that the values outlined in the NHS Constitution are adhered to daily and any matters of concern are raised with the relevant Line Manager or through the necessary processes within the Trust - Responsible for delivering a compassionate, dignified and respectful service to staff, patients and their carers at all times.

Main duties of the job

To provide professional advice to the Executive Nurse and Executive Directors

on all matters relating to end of life under the guidance of the clinical lead.

Responsible for ensuring that every team involved in the care of people at end of life is supported to be aware of recognition and diagnosis of end of life to enable individuals to live well and die well through a person centred approach with shared goals.

Ensure that all staff delivering services in EPUT understand the principles of what constitutes as end of life care and are fully aware of the support available to people with life limiting illness. This will include promoting awareness and discussion of issues relating to end of life care, dying, death and bereavement.

Lead on and evaluate initiatives aimed at improving and developing services for patients and families utilising the end of life care dashboard and audit programmes with qualitative measurement standards. Ensuring that resulting recommendations are embedded in clinical practice, working closely with clinical governance and quality team to achieve this.

Support the systems for reviewing incidents related to end of life; ensuring learning is shared across services and create action plans to measure outcomes and learning from incidents.
