Job summary
The role of the Enhanced Support Team Nurse Associate is to support the nursing team in providing holistic and effective nursing care to those people with learning disability requiring the support of the Learning Disability Enhanced Support team. The post holder will work with the team and other partner agencies to support those people with learning disabilities as risk of admission to mental health or learning disability assessment and treatment units owing to mental health or behaviour decline. They will support the team to work within Building the Right Support and assist with the team responsibilities in maintaining the Dynamic Support Register.
Main duties of the job
- The post holder will work in close collaboration with other professionals, including the wider MDT to assess service user needs, devising care plans and evaluating outcomes that promote service user empowerment through the process.
- Work as a practitioner with interest in mental healthcare and behaviours that challenge in people with learning disabilities. Contributing to national initiatives to promote learning.
- Support the Advanced Practitioner in the development of the Enhanced Support Team.
- Support the smooth running and coordination of the service and monitoring service activity.
- Be responsible for maintaining clinical standards under the direction of the Advanced Practitioner.
Work in liaison with services users, carers, other professionals ensuring the delivery of a needs led service
