Job summary
An exciting opportunity has arisen to join the nurse led integrated community respiratory team delivering care to the adult population of South East Essex.
The team have two sites, Rochford Hospital and Thundersley clinic and are looking for associate practitioners to support in delivering respiratory care to patients in a clinic setting or their own home.
The team consists of B7 specialist nurses and physiotherapist, B6 nurses and physiotherapist, B4 associate practitioners, B3 HCA and admin support. As an integrated service the team provide assessment and review of patients with a confirmed respiratory diagnosis including COPD, ILD and bronchiectasis, diagnostic spirometry, oxygen as the local HOSAR and respiratory physiotherapy.
The successful applicant will have ideally completed the associate practitioner FdSc course in addition to community experience which would be an advantage alongside respiratory experience with a good understanding on long term conditions and multi morbidities. Excellent communication and listening skills is essential.
The successful candidate will demonstrate an ability to work autonomously as well as part of a team and have an understanding of primary care, secondary care, MDT working and ICB's.
Main duties of the job
To assist the team in the management and service delivery for local residents suffering from respiratory disease. This will require holistic assessment to meet health and social care needs and will include assisting with respiratory virtual ward, performing basic observations, venepuncture, capillary blood gases and undertaking spirometry.

The post holder will hold the Assistant practitioner qualification and will ideally have respiratory and organisational experience. The post holder will be professionally and legally accountable for all aspects of care relating to clinical areas and responsible for allocated patients.
The role will include reassessment of respiratory patients as identified by the trained nurse, performing a variety of tests and clinical observations.
The post holder will manage change, monitor and improve standards through supervision, evidence based practise, clinical audit, research and education. You will promote and demonstrate best practise by integrating evidence into your practise.
Excellent interpersonal and communication skills are essential as the post holder will be expected to work collaboratively, actively supporting and liaising with other health and social professionals from both acute, primary care and other agencies.
This role has a strong emphasis on assisting to develop breathlessness hubs and spirometry will be an integral part of the hubs. The post is within the established respiratory team in South East Essex and will offer clinical supervision and teaching.
Promote effective self-management of patients and/or providing professional intervention for community management in order to prevent unnecessary hospital admissions and improve quality of life.
To contribute to the improvement and development of the oxygen/respiratory service.
