Job summary
This is a new role as part of the continuous development of the integrated mental health neighborhood service within West Essex. West Essex was an early implementer in the delivery of transformed community mental health services. The services have been developing over the last three years and are successful in their aims of providing improved access and interventions to the West Essex population. The service is always looking to continuously improve. The role of the clinical manager will therefore provide day to day clinical management of the service and support ongoing improvement in collaboration with our local population to meet the needs of West Essex residents.
Main duties of the job
Post holder will be involved in Service Transformation assisting the Senior Management Team and Key Stakeholders to support continuous development and improvement. You will be responsible for supporting and managing the integrated neighborhood model of care working within the West Essex Primary Care Networks.

Job responsibilities
- Deliver the service model for the West Essex Health and Care partnership integrated PCN Neighbourhood model.
- Responsible for the overall delivery of a range of evidence based interventions
- to service users based on identified needs utilising agreed patient reported outcome measure tools.
- Ensure service users are seen in a timely manner by the right person, at the right time, in the right location.
- Implement a team approach to case management where practitioners are not isolated in decision making and have the ability to access a range of expertise to inform decision making.
- Work collaboratively with Consultant Psychiatrists and Senior Clinicians within the West Essex care unit.
- To take responsibility for supervising and managing clinical activity and casework
- Develop robust interagency relationships with key stakeholders and other outside agencies supporting the development of the integrated care pathways and locality working.
- Ensure the efficient and effective operation of the service, managing budgets, matching resources to need, and coping flexibly with workload pressures.
- Work collaboratively with the West Essex care unit corporate support services to establish and manage operational systems which highlight early warning of deviation from financial / activity targets, and initiate corrective action.
- Assist in planning and managing the implementation of projects to improve facilities and working practices, as agreed with the Operational Service Manager.
- Ensure through implementing effective performance management that all key performance indicators and targets identified by the Trust are met.
- Responsible for the coordination and management of the team in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high quality care and support.
