Job summary

We are seeking to recruit a band 6 Community Mental Health Practitioner in our Dementia Service. We have a commitment to provide responsive, flexible and evidence based specialist service to those people who are undergoing an assessment of their memory problems or those diagnosed with a dementia offering assessment to their complex presentation and needs.

You will be working as a member the Dementia intensive support service to provide high quality, evidence-based, holistic care to patients in line with your professional code of conduct and Trust policies. You will assess, plan, implement and evaluate packages of care based upon individual service user needs, under the guidance of the Advanced Nurse Practitioner.

As required by your manager, you will have responsibility for the day-to-day supervision and management of junior team members within your professional group and team locality. This will include the management of these staff in line with agreed Trust HR processes.

Main duties of the job

As part of the ongoing transformation of services in line with the NHS Long Term Plan, development of services for dementia have been working over the past year in developing an intensive responsive service to manage the needs of these patients. The role is to join EPUT as a Band 6 Senior Community Mental Health Practitioner within the Dementia Service which includes the Memory Assessment Service, Dementia Intensive Support Team (DIST) and the Dementia Support Team (DST).

The Memory Assessment Service offers age inclusive memory assessments and diagnosis. The Dementia Intensive Support Team provides short-term intensive treatment in the person's place of residence working towards admission avoidance. The remaining community teams provide support in enabling people with dementia of all ages to maintain their independence in their preferred place of residence looking at a complete holistic assessment leading to better health outcomes. This includes access to other community services that include assistive technology, advice about environment, and healthy living. Providing education for their families, friends and carers so they are better equipped to understand and support their loved ones with dementia

Undertake assessments with people deemed at high risk due to behavioural and psychological symptoms of dementia. Assessments occur in a range of settings but mainly in the individuals place of residence

Following assessment you will take a lead role with the implementation and evaluation of person-centred care being mindful of physical health and social inclusion to avoid unnecessary hospital admission and to promote early discharge. .

Advise a range of health and social care professionals on effective treatment, interventions and management of individuals with dementia promoting independence.

Signpost individuals to other appropriate services whether they be statutory or non-statutory.

Ensure effective lines of communication are maintained and establish to promote effective working relationships.

Disseminate specialist knowledge across a range of professionals and services as required.

Actively and constructively contribute in Team Meetings, MDT meetings initiating and discussing new methods/models of working.

Participate in research or trails where necessary or required.

Inform the Team Manager if they are concerned with regards to any breaches of professional conduct.

Have a sound knowledge/understanding of the legislation as part of the role of the Registered Mental Health Nurse or Registered Nurse, including Mental Health Act 1983, Mental Capacity Act, 10. In association with Anglia Ruskin University, University of Essex ensure the learning environment for all nurses in training is appropriate and that teaching areas comply with all N.M.C. Regulations. Participate and take professional responsibility/accountability and ensure that accurate, timely records are maintained in line with the Trust Policy.

Ensure that clinical practice is evidence-based wherever possible and participate in research and audit.

Following any serious incident ensure that required documentation is fully completed in line with Trust Policy and Procedures.

Be responsible for updating own knowledge of contemporary psychiatry and a commitment to continuing own professional and personal development, in line with Trust Appraisal and Development Policy

Attend training as provided and be responsible for keeping yourself up-to-date at all times with mandatory training.

Undertake and be proactive in clinical and managerial supervision as required and identify the training needs of junior, unregistered staff and students.

Participate in the appraisal process every year; ensure all personal objectives are met in a timely manner

Promote a positive image, showing a professional and helpful approach and identify where relationships with the public could be improved, in line with the Trust customer care agenda
