The post holder will play a key role in supporting the transformation of local services and the development of integrated practice for the benefit of the local population. They will do this by facilitating joint working between professionals and organisations that result in the delivery of more effective care. You will be enthusiastic and an enabler; you will be a vital presence within the PACT and neighbourhood team environment and instrumental in the effective delivery and maintenance of models. You will be a flexible resource for the workforce.

Main duties of the job

You will be responsible for the design and delivery of change programmes to enable the achievement of agreed goals and expected benefits, connecting with staff to facilitate and prepare them for the transition to new ways of working.

You will lead and arrange workshops with relevant levels of staff to develop new business processes as required. This will include developing workshop material including presentations to facilitate discussions with staff and reach agreement on how new processes should work. The role will involve ensuring that identified governance controls are met where changes are introduced

To inform future evolution of the PACT model and Integrated Neighbourhood Teams and ensure that it develops in a way that brings the greatest impact on outcomes, and benefits to workforce and residents. This will include engaging with Clinical Directors, GP's, PCN employees, patients, community care providers, third sector organisations, and all locality stakeholders. Responsibility will include influencing all relevant stakeholders at neighbourhood level to see the value in the PACT model and Integrated Neighbourhood Teams and to shift their operation towards greater integration and collaboration working.

Working closely with a range of colleagues across Mid and South Essex using knowledge of information systems and information flows to provide analysis and insights to help resolve current challenges.
