The North Uttlesford Integrated Community Team (Saffron Walden) are looking for a dynamic, innovative compassionate Band 5 nurse who wants to deliver high quality healthcare to patients within their own homes.
This is an exciting opportunity for an experienced registered Nurse to work as a Band 5 as part of our friendly and dynamic multi-disciplinary integrated care team. The community nursing and therapy team are are an integral part of the PCN Aligned Core team (PACT) within the North Uttlesford Primary Care Network, working collaboratively with the GP's, Social Care, Out of Hospital Model and voluntary sector to deliver safe, timely care to the local population.

Main duties of the job
Take responsibility for a patient or a defined group of patients within their own homes or elsewhere as defined by the Senior Nurse.
Undertake comprehensive nursing assessments to identify the patients's physical and environmental needs.
Plan, evaluate and priorities interventions and care for patients within his/her sphere of responsibility.
Regularly review caseload patient care, informing the Senior Nurse of any changes made.
To act up in the Senior Nurses' absence.
Responsible for the delivery of nursing care for a geographical area including weekends, evenings and bank holidays.

- The post holder will be responsible for assessment of care needs, developments, implementation and evaluation of programme's of care for patients within the Team Leaders sphere of care.
- The post holder works without direct supervision and is responsible for nursing care for the geographical area including weekends, evenings and bank holidays.
- The community team of West Essex is part of West Essex Integrated Care System supporting Out of Hospital Care to support West Essex Residents. The post holder will be working as part of the Primary Care Aligned Team (PACT) supporting North Uttlesford (Crocus Medical Practice, Gold St Surgery, Thaxted Surgery and Newport Surgery) Primary Care Networks with a focus on prevention of admissions and facilitating hospital discharges.
- Takes responsibility for a patient or a defined group of patients within their own homes or elsewhere as defined by the Senior Nurse.
- Undertake comprehensive nursing assessments to identify the patients physical and environmental needs.

- Leads the clinical nursing management of the caseloads most dependent patients. Determines, in liaison with the Senior Nurse the frequency, type and monitors the efficacy of patient care offered.
