Job summary
Due to a new vacancy being created, we are looking for an enthusiastic nurse, social worker or allied health professional to join our growing Care Coordination service. Focussing on frailty, we assess, treat, signpost and educate people experiencing frailty and how they access health and social services.
The role enacts a holistic approach to ensure that patients are safe and comfortable in their own homes as well as their families and carers.
Main duties of the job
- To co-ordinate the pathway of care for vulnerable older people, identified for case coordination across all service interfaces. This will involve managing a varied caseload and working with and across agencie
- To actively engage and have a regular presence with the GP surgeries and PCN's, supporting them with proactive identification of patients, frail and at risk of admission/loss of independenc
- To be an active participant at monthly GP/PCN MDT's highlighting patients in anticipation of triggers or perceived crisis, ensuring a proactive plan is in place earlier rather than late
- To actively track patients going into hospital known to the team, forge strong links with secondary care, and follow the patient's home on discharge
- Within discharge to assess / visit clients at home within 2 days of hospital discharge, conducting a holistic assessment of their physical, social and psychological need
- To ensure that the patient has an individual care plan with focused goals central to the care coordination process
- To support the use of frailty tools and risk assessment tools across the wider teams
- To use a range of verbal and non-verbal communication tools to communicate effectively with patients, carer's families, and colleagues.
- To refer the client to other services as required.
