Junior Children's Community Nurse Band 5 19.5hrs
We are looking for a highly motivated, innovative and flexible Children's Nurse to join our expanding community team. The successful candidate will be an RSCN or RN Child with acute paediatric experience; they will have an in-depth knowledge of providing family centered holistic nursing care .The individual will be organised and be an excellent communicator able to cross boundaries with the MDT and families alike.
The role includes collaborative working with Health Visitors, School Nurses, GPs and the Paediatric Liaison Health Visitor to ensure the delivery of relevant, co-ordinated and high quality care solutions for children and families within the home setting.

Main duties of the job
- The CCN is responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care, initially with supervision from designated preceptors, progressing to without supervision and will be required to teach other registered and non-registered nursing staff.
- The Children' Community Nurse (CCN) is a member of the Children's Community Nursing Team providing specialised and skilled nursing care of children in the community as designated by the Modern Matron/Team Leader of the CCN service. To reduce unnecessary hospital admission and promote hospital avoidance and to provide specialist nursing support in the child's own environment in partnership with the child, family, carers and other professionals.

- As a registered nurse, she/he is accountable for the care she/he gives and is expected to comply with the NMC statutory obligations, competencies and code of conduct.
- The CCN will be expected to work initially with supervision from designated preceptors progressing to without direct supervision on the understanding that the modern matron/CCN Team Leader acknowledges responsibility for all delegated care.
- The CCN will be responsible under supervision for developing time management skills and be able to escalate difficulties and issues with both the modern matron/team leader
- The CCN will be a junior member of the generic CCN team expected to consolidate nursing skills and knowledge under preceptorship leading to the development of the advanced nursing skills and knowledge required for the development of the role.
- The CCN will recognise his/her own realistic workload and discuss this with modern matron/ team leader/Deputy.
- The CCN to be aware of and use the ancillary services that are available to children, maintaining close links with such services as appropriate.
- To support and work with all health, social care, local authority and third sector providers to ensure the care planned and provided, is done under the banner of integrated working, this includes the sharing of records within the remit of policy.
- To follow all Trust policies, guidelines, procedures and protocols.
- To attend case conferences as appropriate linking with the modern matron/Team Leader.
- To participate in flexible working to provide service provision to children and their families.
- Meets with the modern matron/ team leader as per policy to discuss clinical issues.
- Be aware of and responsible for the escalation and reporting of safeguarding issues as they arise and in timely manner directly in line with the safeguarding policies.
