Job summary

The post holder, under supervision and direction of the Community Nurse will carry out delegated tasks involving direct patient care. You will be part of a community nursing team providing a patient focused service committed to deliver high quality care using a multi-disciplinary approach.

The West Essex community team is part of the wider Uttlesford & West Essex Integrated Care System supporting Out of Hospital Care for West Essex Residents. The post holder will be working as part of the Primary Care Aligned Team (PACT) supporting the Saffron Walden and the surrounding areas, Primary Care Networks with a focus on prevention of admissions and facilitating hospital discharges.

If you have been successful at the interview stage, but do not have enough experience for this role, you may be offered a position as a Band 2 Heath Care Assistant Apprentice. As part of the Band 2 HCA role you will be enrolled onto the Apprenticeship scheme, providing you with the opportunity to earn as you learn. This role enables you to gain valuable experience on the wards/community settings whilst working towards gaining your Care Certificate and naturally progressing into the guaranteed Band 3 HCA role.

Healthcare Assistant

We are seeking enthusiastic Healthcare Assistant with excellent communication skills, compassion and empathy to join the Essex Partnership University Foundation Trust NHS family and work as part of the North Uttlesford Integrated Service, working within the Community Nursing Team.

The team supports West Essex Primary Care Networks across Uttlesford . Your role as part of the team will be integral in supporting the local population for those who have complex needs and long term conditions.

You will be visiting people in their own homes to enhance the delivery of patient care and to provide efficient and effective service.
