Job summary

Associate Practitioners are fundamental to the delivery of integrated care, working collaboratively with therapy, medical and nursing staff to support and facilitate the treatment of patients within their own homes.

The Associate Practitioner will work as an active member of the Community nursing Team in providing a high quality, efficient and effective service to patients.

The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of a registered practitioner. They will work within relevant legal and ethical frameworks in accordance with organisational protocols and policies.

The Associate Practitioner will be proficient, knowledgeable and competent. They will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of the patients, the service and the team.

Main duties of the job

Community Healthcare Assistant Band 4

Then join our dynamic and forward thinking integrated nursing team and make an important contribution to the delivery of high standards of care to patients, their families and carers within the South East Essex locality.

The successful candidate will provide support in order to prevent patients having unnecessary hospital admissions, enabling them to stay at home where possible and work with wider integrated community teams to facilitate and support patients in their own home.

Applications are welcomed from HCA's with a minimum of three years community experience or who hold an Associate Practitioner qualification.

This is a full time position - Various shifts 8am-9pm Mon-Sun
