Clinical Support Worker

An exciting opportunity has arisen for a Band 4 Clinical Support Worker to join the Newham Health Team for Adults with Learning Disabilities (NTPLD), as a permanent, full time (37.5hpw) contract. The Clinical Support Worker will be supporting people with a Learning Disabilities, preparing for and attending Annual Health Checks, assisting with GP/Hospital appointments, working alongside the Learning Disabilities Primary Care Liaison Nurse to ensure that people have equitable access to cancer screening (and other health) programmes with reasonable adjustments.

Main duties of the job

The Primary Care Liaison Service works closely with GP's to identify individuals with a learning disability and provide additional support, such as helping to arrange appointments for people with special requirements by promoting reasonable adjustments. They also work closely with people with learning disabilities and their families and carers' with the aims to improve health outcomes and reduce health inequalities.

This post is suitable for someone who is passionate about primary health care and promoting Annual Health Checks, by ensuring a high quality holistic approach to the health care and support provided to people with learning disabilities. You need to be highly motivated, imaginative and committed to improving health outcomes and challenging barriers to healthcare for this population.

The post is hosted by the East London NHS Foundation Trust, but works in close partnership with the CCG and local authority. The post is based within the NTPLD offices, but travel is required to other parts of the borough. You will be required to work autonomously, as well as part of the wider team, with a variety of agencies outside of the Trust. As the Clinical Support Worker, you will be clinically and operationally supervised by the Band 7 Health Facilitation Nurse Lead

The Trust actively encourages professional development through the appraisal process and regular supervision, and you will be offered support to develop your clinical skills within an established team.
