Rehab Support Worker

Job summary

An exciting opportunity has arisen for a confident proactive individual within Tower Hamlets Community Health Teams. An experienced rehabilitation support worker is required to assist Community therapy team in providing initial assessment of patients within set parameters, supporting therapists in organising and running patient exercise groups and implementation of rehabilitation programmes in order to facilitate clients reaching their optimum level of independence and social integration.

The service has a strong and well established training and education programme, along with robust personal development review programme to support staff with their learning and development.

Main duties of the job

General Duties and Responsibilities

  1. To support the department and organisation by carrying out any other duties that reasonably fit within the broad scope of a job of this grade and type of work.
  1. To have responsibility for the health, safety and welfare of self and others at work.
  1. To represent the profession and the Trust in a positive and professional manner at all times, contributing personally and professionally to the development of the Community Health Services in East London Foundation Trust to the best of your ability.
  2. To comply with the requirements of the Data Protection act
  1. To comply with the trust's corporate and local policies and procedures on such subjects and clinical management and human resources
  2. To ensure that patients, clients and members of the public are consulted and involved in decisions about local health service developments.
  3. To undertake such other duties as may be required from time to time and are consistent with the responsibilities of the role and as qualified and able

  • NVQ 2 Equivalent
  • Experience of working with older people in a community rehabilitation setting
  • Experience working with older people or people with disabilities
  • Experience of client centred working, within a multidisciplinary team
  • Good general secondary school education (English, Literacy and Numeracy Skills)

Desirable

  • Car owner/Driver
  • Manual handling techniques
  • Word-processing/IT skills

Knowledge/Skills and experience

Essential

  • Ability to assist clients who are frail or who requires assistance with activities of daily living and to apply a rehabilitation approach in a support worker role.
  • Ability and willingness to work across different sites and within the community (e.g. client's homes, workplaces).
  • An understanding of the needs of Tower Hamlets culturally diverse population
  • Understanding of health and safety issues in relation to working in a client's home (trips and falls, verbal/physical aggression)
  • Equal Opportunities related to staff and client
  • Ability to manage own workload, work flexibly and use initiative to problem solve.
