Community Rapid Response Nurse

Job summary

Are you a Band 5 Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced part time Band 5 nurse to support the changing face of community health care within our Rapid Response Team on a fixed term basis to cover maternity leave. As part of the Urgent Community Response and Virtual Ward initiative, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect.

These posts will be to cover our North Bedfordshire Team based from Twinwoods Health Resource Centre, Clapham, Bedford.

Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.

Main duties of the job

You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Consideration will also be given to staff who are newly qualified and are passionate about a career in community nursing. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.
