MAIN DUTIES AND RESPONSIBILITIESPatient Care Practice autonomously and take professional accountability for all aspects of patient management. Ensure high standards of clinical care for patients under your management and those you supervise or assess. Prescribe safely and effectively within your sphere of competence. Stay clinically updated and credible as a practitioner to maintain best practices. Provide high-quality, compassionate care, respecting patient dignity and individual needs. Support patients and families with clear information, education, and reassurance throughout their care journey. Work collaboratively with the multidisciplinary team to develop and implement personalised care plans.Clinical Act as an expert practitioner, integrating research evidence into practice through advanced clinical reasoning and decision-making. Assess, examine, investigate, diagnose, and treat patients with mental health conditions, ensuring safe management, referral, admission, or discharge. Make independent clinical decisions, initiating appropriate invasive and non-invasive investigations, treatments, and interventions. Plan, order, interpret, and act upon test results, as well as other clinical and non-clinical information. Prescribe and administer medication as appropriate, using independent prescribing qualifications or patient group directions. Deliver evidence-based clinical interventions, assessing and monitoring patient progress while escalating concerns when necessary. Ensure compliance with clinical guidelines, policies, and procedures to maintain high standards of patient care.Administration Ensure that accurate documentation and records of patient care are maintained in a timely manner. Promote dynamic, innovative, and continuously improving practice that meets the evolving needs of the population. Provide supervision, mentorship, and peer support to junior staff, fostering professional growth and reflective practice. Support staff recruitment, induction, and ongoing professional development to build a skilled and motivated workforce. Participate in clinical supervision, seek peer review, and use feedback to guide personal and professional development. Assist in risk management by ensuring adverse incidents are reported, investigated, and used to promote a learning culture. Contribute to service improvements, patient flow initiatives, and integrated working with primary and secondary care colleagues. Foster a positive, inclusive, and supportive workplace culture that encourages collaboration and continuous learning.Performance and Quality Demonstrate excellent consultation, physical examination, and critical thinking skills to assess, diagnose, and manage patient care effectively. Actively engage in student nurse development, serving as a practice supervisor/assessor and fostering a culture of learning and openness. Ensure prescriptions comply with legislative and best practice requirements, prescribing in line with NICE guidelines and Trust policies. Analyse and interpret complex clinical information, reassessing patients to evaluate treatment effectiveness and adjust care plans accordingly. Work collaboratively with the multidisciplinary team to ensure evidence-based care and continuous quality improvement. Contribute to service development by benchmarking against best practices, implementing innovations, and improving patient outcomes. Investigate incidents and complaints in line with Trust policies, making recommendations for service and practice improvements. Participate in audits, risk assessments, and clinical governance initiatives to drive continuous service improvement. Promote patient safety and safeguarding, addressing individual needs, including child protection and vulnerable adult concerns. Act as a positive role model, raising concerns about care delivery, risk, or incidents sensitively and constructively.Financial and Physical Resources Support the budget holder in delivering care within financial constraints and available resources. Ensure efficient use of financial and physical resources, contributing to cost-effective decision-making. Maintain a personal duty of care in the safe use of equipment, ensuring best use of resources. Identify the need for diagnostic tests, interpret results, and apply specialist clinical knowledge in decision-making. Perform advanced physical assessments and independent prescribing in line with Trust policies. Obtain informed consent from patients for treatment, investigations, and management. Work flexible hours, including out-of-hours and weekends, to provide advanced clinical skills and leadership. Adhere to health and safety regulations, including moving and handling, to protect self, patients, and colleagues. Travel between community locations as required to meet service needs.
